Introduction



Saamis Immigration Services Association (SISA) is seeking a motivated and dedicated individual to join our team as a Case Management Coordinator on a 5-month contract. This role presents an excellent opportunity to contribute to meaningful work that supports newcomers in our community, while building valuable experience in a collaborative and mission-driven non-profit environment.

Summary:



As a Case Management Coordinator, you will play a key role in delivering a client-centred case management service model to support eligible clients with their settlement in Medicine Hat primarily. The Case Management Coordinator provides intensive case management tailored to the complexity of the clients' barriers and needs, including regular check-ins with the clients, appropriate referrals, and follow-ups. You will be part of a team that values diversity, inclusion, and community impact.

This is a temporary position designed to provide additional support in meeting service demand. While the term runs until March 31, 2026, there may be potential for extension depending on organizational needs and funding.

Primary Duties and Responsibilities



Office Administration



Maintain thorough documentation of client cases according to nationally established standards using Better Outcomes and ICARE databases.

Prepare and submit monthly, quarterly and annual service and statistical reports according to the program and funder requirements.

Record and report every service provided and time spent with each client weekly, monthly reports and a stats sheet.

Attend professional development workshops and conferences as requested by management.

Provide monthly reports to the Settlement Manager.

Open and maintain clients' files up to date.

Case Management Services



Conduct initial intake and needs assessments using the needs assessment matrix, quarterly check-ins, empowerment plans, and ongoing needs assessments.

Provide case management services according to the client's level of need through assessments, information orientations and referrals, accompaniments, home visits, follow-ups and advocacy to assist clients in living independently and integrating successfully using anti-oppressive and empowerment-based practices.

Provide necessary interpretation and translation for direct service delivery to clients during community appointments.

Conduct regular home visits and accompany clients to various community appointments for different services and resources.

Provide life skills, telephone support, and crisis intervention as needed.

Participate in case management meetings with internal staff and external service providers.

Coordinate and facilitate program activities as required.

Assist with monitoring and evaluating the program on an ongoing basis, recommending service delivery changes and assisting with their implementation when necessary.

Participate in the ongoing consultation with the Settlement Manager.

Community



Across Medicine Hat, provide information, orientations and referrals to various community agencies, professional services, settlement and social services, etc.

Liaise, collaborate, and advocate with internal programs and external service providers/community agencies to secure services and resources that bridge the gaps in support needed for this population.

Partner with other SISA departments, community agencies, and other service providers related to the settlement process.

Maintain and enhance positive partnering with fellow agencies.

Other



Participate in monthly staff meetings, training sessions and special events organized by SISA.

Provide settlement and other program support when needed.

Other administrative responsibilities as assigned by the Executive Director and Managers.

Qualifications and Experience:



A Bachelor's degree in Social Work, Social Services, Counselling, Psychology, Human Services or related fields is required. A Master's in related education will be preferred. A minimum of 2 years of relevant practical experience working with an appropriate population is essential. The ability to speak other languages is considered an asset. Fluency in oral and written English is required. Minimum one year of experience in an Intensive Case Management role serving a vulnerable population, including trauma victims, is an asset. Proven ability to work effectively with newcomer clients from diverse cultural backgrounds is an asset. Knowledge of settlement services and the integration process, as well as a working understanding of the challenges newcomers face in Canada, are assets. Extensive knowledge of community programs, services, and resources is required. Experience in team building. Possess excellent written communication, organization, time management and coordination skills. Must have a positive and collaborative attitude, demonstrate flexibility, reliability, and proactiveness, and take the initiative. Demonstrate the ability to multitask and thrive in a fast-paced environment and high-stress situations. Advanced computer skills and strong abilities using the following programs: Microsoft Word, PowerPoint, Excel, Outlook, Adobe and program databases. First Aid/CPR training is an asset. Available to work flexible/irregular hours, including evenings and weekends. A driver's license and access to a vehicle for work purposes are a vital asset.

Health and Safety:



Work in compliance with the OH&S Act and Regulations and abide by the SISA's health and safety policies and procedures. Ensure that the equipment, materials and protective devices prescribed and regulated (OHS) are provided and maintained in good condition. Apply health and safety regulations and policies. Maintain a safe working environment.

Accountability:



Adherence to the policies and regulations of SISA. Accurate and timely reporting. Performance appraisal and ongoing quality control by the Executive Director.
