JOB OVERVIEW
This is an exciting opportunity to work in Canada's first Indigenous-Led RAAM Clinic. Learn about Indigenous healing practices and discover new ways to incorporate this energy alongside the western practices of healthcare service in order to deliver culturally safe medicine.
Under the general direction of the Site Coordinator and Clinical Team Leaders, the Crisis Clinician Nurse will utilize their nursing knowledge, skills, and judgment to provide optimal care to patients accessing services. This will include mental health and addiction related assessments, interventions, and treatments.
The crisis clinician will practice autonomy and will formulate clinical decisions and development of a patient care plan in collaboration with the multidisciplinary AHWC / RAAM team. Drawing from the strengths of this diverse team and from AHWC's values and mission statement, this role requires a welcoming, kind, and compassionate person for community members who visit the Indigenous-led RAAM Clinic to access supports during their healing journey.
The Crisis Clinician Nurse should demonstrate an understanding of and respect for the histories, traditions, and cultures of Indigenous peoples, inclusive of First Nation, Metis, and Inuit Communities and Nations, including an understanding of the importance of decolonizing healthcare systems and spaces.
EDUCATION
RN/BN or BSc.PN required. Education and training experience in substance use / dependency), an asset
ESSENTIAL SKILLS
Four years related community experience

MAJOR RESPONSIBILITIES AND DUTIES
(as per AHWC)
Welcomes and supports all Indigenous - First Nation, Inuit, and Metis- and non-Indigenous community members entering AHWC / RAAM Clinic, including people who identify as part of the 2SLGBTQIA+ community and persons who live with disabilities; sees to their immediate comfort
Nurtures a judgement-free, shame-free environment, focused on harm-reduction and the journey of healing that the individual has defined for themselves
Emphasizes culturally sensitive, culturally relevant, person-centered care
Delivers safe and clinical practices in accordance with: AHWC's program standards, policy and procedural operations, and organizational values; RAAM's program standards; Regional Health and Provincial protocols
Gives community members hope and encouragement
Assists in response to emergencies by being familiar with the fire plan, emergency procedures, safety protocols, and location of emergency equipment, etc.
Responds to emergency codes as per procedures
Works in collaboration with Site Coordinator for RAAM Clinic and reports operational issues as advised
Attends staff meetings as required
Meets with supervisor as required
Adheres to instructions respecting security, use, storage, and destruction of the Personal Health Information (PHI) Act
Possesses knowledge of social issues such as poverty, homelessness, food insecurity, substance use, mental health/illness, racism, and other challenges faced by Indigenous populations in Winnipeg
Completes other duties that may be assigned (be a ' Helper*') when required as part of the RAAM Clinic team, not exceeding stated skills and capabilities
MAJOR RESPONSIBILITIES: CRISIS INTERVENTION and ADDICTION SUPPORT
1. SCREENING
Indicators:
The Clinician
Engages client in a manner that builds trust and utilizes a trauma informed lens
Provides a brief screening function for the purpose of establishing priority for clinical intervention and the appropriate pathway of care within AHWC
As part of the screening process takes client's vital signs, and escalates concerning values
At all times in working with the client and family, formal or informal supports that are present are welcomed and included in the process as appropriate
Reviews existing information on client including any plans
2. ASSESSMENT
Indicators:
The Clinician
Collects complete and accurate data and prioritizes identified needs
Evaluates potential safety risks and child protection issues and addresses these concerns and a plan of intervention. Appropriate agencies are accessed as needed.
Explores and assesses the resources, psychosocial and environmental stressors that are impacting the crisis situation
Performs a mental status assessment using the approved tool for crisis services; identifies signs and symptoms of mental or physical illness/distress
Completes withdrawal assessment tools such as CIWA, COWS etc. to help determine an appropriate treatment path
Completes a Risk Assessment for Suicide using the ASIST model, when appropriate
Evaluates potential for violence, serious medical complications, victimization, and breakdown of support systems
Determines the next step in the clinical pathway for the individual within AHWC / CSU / RAAM
3. CLINICAL INTERVENTION
Indicators:
The Clinician
Based on the assessment the clinician utilizes the most appropriate clinical intervention
Interventions are chosen in accordance with evidence-based research
Provides crisis intervention to ensure safety, alleviate symptoms, and empower clients;
Facilitates communication and collaboration among clients(s) and their formal/informal
supports to promote resolution of the crisis;
Facilitates effective problem-solving techniques of the client and their formal/informal supports toward resolution of the crisis.
Provides appropriate harm reduction counselling and training as required (e.g. take home Narcan).
4. PLANNING
Indicators:
The Clinician
The clinician collaboratively develops a plan of intervention with the client and other AHWC / CSU / RAAM clinical staff and professionals to minimize risk and alleviate symptoms of distress.
Develops a follow-up plan with the client and support persons that may include referrals to appropriate resources.
The Clinician identifies existing services and incorporates existing treatment plans in the development of the intervention plan.
Evaluates the need for accessing emergency services and facilitates timely access.
Educates clients(s) regarding availability and accessibility of resources.
Advocates for clients in accessing of resources and ensures the links have been established
Completes necessary referral and consent forms, including establishing connection with Primary Care and other identified supports.
5. DOCUMENTATION
The Clinician assumes primary responsibility for ensuring clear and concise reports which detail the nature of the crisis, assessments, the specific interventions and plans for safety and follow-up for each clinic visit, community visit or crisis call.
Documentation complies with the required format, policies, and legal guidelines.
An electronic health record will be utilized.
Information to update a client's status within the AHWC / CSU / RAAM will be maintained and updated.
6. DECISION MAKING AND ACCOUNTABILITY
The clinician is expected to work collaboratively with other team members.
The clinician will be accountable for determining:
The appropriate action to ensure safety of the client and those around them
The creating of effective discharge and transition of care planning. Guidelines to be used for decision making may include, but are not limited to, Policy and Procedures, Mental Health Act, Child & Family Services Act, and the Personal Health Information Act.
7. HEALTH ASSESSMENTS
The clinician/nurse may provide back up for the role of the physician assistant regarding the following tasks:
Does a complete and thorough review of systems, (Health assessment)
Makes decisions about whether the patient is physically well enough to continue with a mental health or addiction assessment
Consults and liaises with emergency departments if needed.
7. COLLABORATIVE CARE
AHWC / CSU / RAAM will be supporting a collaborative model of care to ensure quality care as well as effective utilization of resources. The clinician may at times also provide the same clinical function for individuals that need mobile services in the field, or phone support services.
Ability to seek out, integrate and value as a partner, the input and ongoing involvement of each patient/client/resident/family/community when designing and implementing care and services
Ability to differentiate among one's own role and the roles of other health providers to appropriately establish and achieve patient/client/resident/family/community goals
Ability to apply the principles of cultural humility, team dynamics, and group processes to enable quality patient care and effective interprofessional collaboration to emerge in everyday practice
Ability to apply leadership principles that support a collaborative practice model including shared decision-making and accountability for one's own actions
Ability to consistently communicate in a respectful, responsive, and responsible person-centered manner
Ability to engage self and others, including the patient/client/resident/family/community in a positive manner to constructively address disagreements as they arise
Ability to engage in relationships with care, dignity and respect regardless of race, ethnicity, culture, ability or language proficiency.
Ability to collaborates as required with withdrawal management services and addiction treatment agencies
Adheres to all safety and health regulations and safe work practices.
May be required to perform other duties and functions related to this job description not exceeding above stated skills and capabilities.
