Summary
The Case Coordinator acts as an integral team member in building resilience with refugees and immigrants who have experienced trauma, through coordinated community, social, and mental health support. This position focuses on providing ongoing resource coordination and advocacy through a family centered approach for refugee and immigrant families.

Description
Primary Duties and Responsibilities include but are not limited to:
Facilitate individual and family intakes and collaboratively assess client needs
Coordinate mental health and community support services according to the needs identified by the clients
Provide ongoing case management, case planning, referrals, assessment and interventions
Liaise with program therapists to support the needs of shared clients
Coordinate and participate in case conferences as needed
Liaise with community and agency resources to support clients' social needs through referrals
Develop and deliver client workshops, psychoeducation, and social groups for early intervention and prevention of mental health issues, as need arises
Engagement and coordination of outreach services to schools and community partners
Work collaboratively with a diverse and multi-disciplinary team within CCIS
Practice personal self-care and support the needs of team members
Complete evaluation and reporting of program activities that fit all funder outcomes and indicators as outlined by the Program Manager
Qualifications
Post-secondary degree/diploma in Social Work, Psychology, Education, or related professional expertise
Demonstrated experience in working with culturally diverse children, youth, and families and in working with youth mental health
Ability to work collaboratively and communicate effectively within a multicultural and multidisciplinary setting
Knowledge and competency in cultural diversity and special issues relating to refugees
Ability to work a flexible schedule that includes occasional evenings to accommodate clients
Ability to take initiative, work independently, organize one's work and records
Experience with not-for-profit agencies considered an asset
Experience in managing confidential client and program data
Competent verbal and written communication, computer, and facilitation skills
Ability to communicate in a second language considered an asset
Ability to plan, organize, implement, and evaluate activities
Ability to positively work with challenges
Knowledge of community and health resources in Calgary
Must have a valid driver's license and own reliable transportation.
