Detailed Overview: Provides case management assistance to a trans-disciplinary team who service complex clients and adults with acquired brain injuries. Coordinates services for clients and families. Supports the team with completing various contractual, financial and administrative duties. Responsibilities:
Coordinates contract, financial, and administrative aspects of client services and case management under the direction of managers and clinical staff. Supports clients accessing services or managing urgent issues in the absence of the case manager and ensures that the case manager is then kept informed; reviews files to obtain historical information and reviews materials such as policy and procedures to assist with various case management issues.

Researches community resources and identifies information that will be of assistance in the development and implementation of client services and case management plans. Completes initial intake forms for new referrals to the program, as required. Completes forms and arranges for services with other community agencies and Ministries (e.g. Income Assistance). Completes community based client assessments and assists and/or transports clients to community services. Escorts clients/families to view potential residential placements. Assists in the planning of admissions, discharges, and transfers of clients to residential care placements. Advocates on behalf of clients and families to access system information and resources; promotes client's right to autonomy, dignity and privacy. Organizes client service meetings and schedules community appointments for clinical staff. Works collaboratively with team in the development and documentation of care plans. Completes data entry of care plans and other client service documents, contracts, etc. Monitors quarterly reports and other client service documents completed by Contracted Service Providers. Advises case managers of any deficiencies or concerns related to Contracted services. Participates with the trans-disciplinary team in by providing input with respect to assessment, development, activation, and evaluation of treatment care plans and programs. Maintains client records by ensuring that client files, applications forms, financial documents, quarterly reports, etc, are kept up to date. Maintains spreadsheets and other client or financial information systems. Completes filing and related administrative duties. Performs other related duties as assigned. Qualifications:

Education and Experience



Grade 12, Social Services Worker Diploma and two (2) years' recent experience working with children and adults with physical and/or cognitive disabilities or an equivalent combination of education, training and experience.



Valid B.C. Driver's License and access to personal vehicle for business related purposes.
