Caregiver Educator Dementia Care

Job Description

As a member of the Adult Day and Caregiver Support team the Caregiver Educator is responsible for providing education and support to caregivers of persons living with a diagnosis of Alzheimer's disease or related disorder. This is a full-time position where the incumbent will work Monday to Friday 8:30am- 5:00pm however must have a flexible schedule to include evening work. The individual will be supporting caregivers who work at our Burlington Adult Day program as well as the Burlington and Halton Hills Communities.



Pay range: $52,474.50 - $55, 592 per year


PRIMARY FUNCTIONS:




Educates caregivers, Acclaim Health staff, students, and volunteers on all aspects of disease management. Provides individual and group support to caregivers on progression of illness and the day to day management of illness Contributes to newsletters for caregiver and staff. Conducts home/community visits for support and education to caregivers Develops, coordinates, facilitates, and evaluates caregiver support groups throughout Halton. Plans and facilitates the Dementia Information Series and Discussion Series for caregivers, ensuring caregivers receive accurate information. Develops, coordinates, facilitates and evaluates other educational and support initiatives to meet the needs of caregivers in Halton Region. Plans and facilitates caregiver social and wellness events. Coordinates the annual printing of the Halton Dementia Guide. Supports and evaluate volunteers on an ongoing basis.



QUALIFICATIONS & EDUCATION

:




Baccalaureate Degree, Health/Social Science discipline or equivalent Valid Ontario driver's license, use of vehicle and insurance per Acclaim Health policy.



EXPERIENCE & SKILLS

:




Baccalaureate Degree, Health/Social Science discipline or equivalent Minimum 2 to 3 years' experience working with individuals and caregivers dealing with dementia in a community environment is preferred Well-developed knowledge of dementia. Demonstrated knowledge and expertise in facilitating and presenting to groups. Proficient in Microsoft Office. Excellent organizational skills. Proven ability to multi-task and meet deadlines. Demonstrated ability to work independently and collaboratively as a team member.
Valid driver's license and access to a vehicle

Must be legally eligible to work in Canada
