We are looking for our next new team member to join us in the
unique role of Care Planner
at our
Niagara office
. This role works closely with families in the community, understands care options, complete client assessments, build Care Plans and develop and support field staff.
This is a travelling post - must have a valid driver's license and access to a personal vehicle.
Qualifications:
Minimum of
2 -3 years of Care Planning experience
within the healthcare field
Nursing and/or Clinical Healthcare background
preferred
Strong familiarity with
medical terminology
Proven ability to
multitask effectively
in a fast-paced and dynamic environment
Ability to work
independently
as well as
collaboratively
within a team
Excellent
organizational and prioritization
skills
Strong
written and verbal communicatio
n skills
Exceptional
interpersonal skills
Experience with
InflowCare Inquiry
software and
AlayaCare Home Health Care
software preferred
Proficient in
Microsoft Office
& general computer applications
Responsibilities:
Conduct and document
client assessments,
including in-home visits, and develop individualized Care Plans
Respond to inbound care inquiries and
convert client leads
into active contracts
Create and update customized Care Plans as client needs evolve
Build and coordinate the Care Team, ensuring effectively communication of each Care Plan
Collaborate closely with the Care Coordinators to maintain quality and continuity of care
Oversees care delivery to ensure high standards of quality, consistency, and coordinationof service delivery.
Serve as a key resource for clients and their families, fostering trust and providing ongoing support.
Engage regularly with management, business development and administrative teams to enhance service delivery
Network
with key stakeholders to maintain and develop professional and social contacts (hospitals, discharge nurses, retirement homes, community/national associations - Alzheimer's Association, etc.)
Promote a positive and inclusive office environment through supportive communication and leadership
Contribute to an exceptional client and staff experience t the Right at Home Niagara office
Conduct and facilitate
community presentations and represent Right at Home at local events (fundraisers, charity walks, BBQs, etc.)
Serve as a strong
brand ambassador
for Right at Home, supporting our mission to remain Niagara's leader in home health care services.
