We are currently seeking a full-time permanent Care Planner.
SUMMARY:
The Care Planner is responsible for assessing new and existing clients and developing and monitoring client-centered care plans. The assessment and care planning process may include consultation with family members, caregivers and/or other health care providers. This position has the ability to be a hybrid remote model, with the expectation to travel throughout the community (Huron-Perth) for the completion of home visits.
QUALIFICATIONS:
Qualified RN, RPN, or other related professional designation.
3-5 years related experience, gerontology experience preferred.
Proficiency in use of computer (Word, Excel, Microsoft Office, and typical office equipment).
Demonstrated willingness and ability to be adaptable, build relationships, focus on client needs and communicate effectively.
Excellent interpersonal, communication, typing and time management skills.
Valid driver's license, insurance and use of a vehicle.
Ability to speak French would be considered an asset.
