Corelia Health Inc. is seeking passionate and motivated professionals to join our growing team in a

dual role of Care Planner and Business Development Manager

for our

Halton Hills and surrounding regions

.

We are looking for

Registered Nurses, Occupational Therapists, Physiotherapists, or other healthcare professionals

who are dedicated to providing compassionate, high-quality care while also contributing to community outreach and business development.

Key Responsibilities:

Represent Corelia Health in the

Halton Hills region

as a professional and caring advocate for our brand and services. Respond to client inquiries, conduct

in-home assessments

, and create personalized

Care Plans

tailored to each client's needs.

Recruit, train, and manage

Care Team members while fostering a supportive work environment. Develop and maintain relationships with

healthcare providers, community organizations, and referral partners

to build awareness of Corelia Health's services. Collaborate closely with the Corelia Health management team to support growth, client satisfaction, and regional expansion. Work independently from a

home office

, the

Corelia Health Mississauga office

, and in the

community

--conducting regular visits, presentations, and outreach activities. Use manual/digital tools such as

CRM systems

for client management, scheduling, and communication.

Qualifications:

Background in nursing, rehabilitation, or related healthcare field. Excellent communication, organizational, and leadership skills. Strong commitment to client service and quality care. Valid

driver's license

and access to a reliable vehicle for local travel. Comfortable using computer-based tools and platforms.
