Yorktown Family Services

provides a comprehensive range of mental health services for children, youth and families who are experiencing serious emotional/social/behavioural difficulties along with several gender-based violence initiatives. Yorktown is committed to ensuring that all programs produce effective and positive outcomes for clients through evaluation, research and the incorporation of evidence-based services and practices whenever possible.

In partnership with Moonshotthe Care Navigator role will support gender -based violence programs by engaging with individuals online who are perpetrators of violence, survivors of domestic and intimate partner violence and by standards. The Care Navigator will build relationships online, carry a small caseload and move people from online to offline services across Ontario. In-person work may also be required.

Responsibilities



To act as a single point of contact to integrated care services.

Assess vulnerable clients with complex mental health and social service needs and support referrals to the most appropriate team.

Engage in service coordination and case consultation meetings with partnering agencies and external services to ensure successful service transitions.

Inform program enhancements opportunities to best support evolving needs of communities being served.

Participate in networking opportunities on a local and city-wide level as needed for knowledge exchange and collaboration purposes.

Administer program and process evaluations and data collection for internal and external reporting.

Contribute and facilitate interdisciplinary team meetings and case reviews, producing summaries and gathering recommendations for care.

Present clients at case review meetings.

Bridge clients from various sectors to mental health and social service supports.

Provide triage support for our walk-in clients by supporting them to complete registration forms and providing clients with an initial assessment with a focus on social service support and preparing them for a therapy session.

Provide Care Navigation services for unattached clients who use our Urban Tele-Mental Health Program.

Work closely and within Ontario Health teams, hospitals, and primary health settings.

Participate in planning meetings with Moonshot.

Ensure agency health and safety protocols are followed.

Ensure all relevant documentation/records are maintained and kept in secure location

Additional duties as required

Qualifications



B.S.W. or Social Service Worker / equivalent.

Preference will be given to candidates who are a registered member in good standing with the college of social service workers or college of registered psychotherapists (RSW or RP) or related college.

Minimum 3-5 years relevant working experience in violence against women's sector.

Experience working with women and perpetrators of abuse, with a strong understanding of the psychological and physical effects on women and children considered an asset.

Experience in working in crisis settings or crisis phone/online services considered a strong asset.

Skilled in trauma informed approaches, active listening skills, and crisis intervention, with commitment to supporting safety, empowerment of individuals.

Good presentation and advocacy skills.

Excellent oral and written communication skills, including report writing.

Superb interpersonal and organizational skills.

Demonstrated skills and experience working in collaboration with other service providers.

Ability to work flexible hours including evenings is essential.

Vulnerable Sector Screening is required.
