, with a firm end date. Upon successful completion of the contract, the incumbent would be welcome to apply for other opportunities at Sanguen, based on their qualifications. Should they wish to return, they will be considered an internal candidate for a period of one year following the end of this contract.

Position Summary



The SHHI Care Facilitator will work with the NP, Nurses, Care Facilitators, Navigators, Mental Health and Addictions Counselors of the Supportive Housing and Health Initiative (SHHI) to provide direct, client-centered support to tenants in supportive housing. This role is vital in helping individuals maintain their housing, improve their quality of life, and access necessary services through relationship-building, advocacy, and collaborative support planning. The Care Facilitator will receive referrals and delegated tasks from the SHHI team and will offer health and social system navigation, case coordination and will collaborate with the tenants to develop a Social Support Care plan.

Accountabilities include but are not limited to:



Collaborate with SHHI team members to conduct regular outreach visits to tenants in supportive housing to assess their needs and provide ongoing support. Receive delegated tasks from the SHHI team members that will support the individualized care plans developed in collaboration with tenants With respect for self determination, collaborate with tenant to develop individualized social support care plans Provide health and social system navigation and accompaniment for the purposes of support Provide case coordination Provide crisis intervention, and emotional support to tenants, fostering a trusting relationship. Connect tenants with community resources, including health care, mental health services, addiction support, and employment programs. Work closely with supportive housing staff and other service providers to coordinate care and ensure tenants receive appropriate supports. Advocate on behalf of tenants with service providers, government agencies, and other stakeholders. Utilize a harm reduction and trauma-informed approach in all interactions with tenants. Maintain accurate and up-to-date case notes, service plans, and documentation as required by the program. Participate in team meetings, case conferences, and professional development opportunities to enhance skills and service delivery. Promote a client-centered approach focusing on strengths and resilience in all interactions.

Qualifications



Completion of a degree or diploma in Social Service or Social Work. 2-5 years of experience providing community-based case management from an anti-oppression, trauma-informed framework. Minimum of 2 years of experience working with individuals experiencing mental health challenges or substance use issues. Demonstrated ability to work effectively with individuals from diverse backgrounds, including those experiencing complex trauma. Experience with harm reduction and trauma-informed care principles. Strong interpersonal and communication skills, with a demonstrated ability to build rapport and trust with clients. Ability to work independently, manage time effectively, and adapt to changing priorities. Proficiency in crisis intervention and de-escalation techniques. Understanding of anti-oppressive practices and commitment to equity, diversity, and inclusion. Experience in advocacy and system navigation for marginalized populations. Experience working with racialized populations, people original to this land Indigenous peoples, people who engage in sex work, people with current and former involvement in the criminal justice system, people with mental health needs, those living with trauma and people who use drugs. Experience working with people who use drugs, as service users and as colleagues.

Hours and Location of Work



Working hours will be 40 hours per week, primarily daytime hours with some evenings and the possibility of occasional weekends as demanded by the nature of the work.

The primary place of work will be at Supportive Housing locations throughout the Region of Waterloo. Regular travel is required within the Waterloo Region either by use of a personal vehicle or regional personal transit pass.

Compensation & Other Perks



The compensation for this role will range from $27.88 - $29.81/hour ($58,000 - $62,000/year) depending on experience and qualifications. 4% vacation pay. 15 paid sick days per calendar year (accrues biweekly).
