Reporting to the Clinical Manager, the Care Facilitator provides leadership and organizational support to ensure effective and efficient day to day operations and optimize patient flow on the clinical unit.
The Care Facilitator supports the nursing staff to work collaboratively with the interprofessional team to enhance patient care and supports the nurses to foster a culture of continuous performance improvement.
Assign patients to appropriate care giver to meet patient care needs
Adjust staff workload in response to changing unit conditions
Provide support to nursing staff as they deal with unexpected unit events, and family needs
Respond to patient complaints and collaborate with the health care team to resolve them
Collect data regarding standards of care on the unit, incident reports, near misses, waiting times to be reported through the appropriate channels in order to improve patient care
Participate in continuous performance improvement and unit audits
Demonstrate QCH values: collaboration, accountability, innovation, respect
REQUIREMENTS:
Registered with the College of Nurses of Ontario (CNO)
Bachelor of Nursing
Minimum of 3 - 5 years recent Emergency / Critical Care nursing experience
Excellent communication skills (oral and written)
Excellent organizational, interpersonal, problem solving and decision making skills
Demonstrates knowledge and ability in conflict prevention and resolution
Demonstrated leadership and organizational abilities and is able to apply in a clinical setting
Demonstrated computer competency
PREFERRED:
Certificate in Emergency / Critical Care Nursing
Registered Nurses Association of Ontario (RNAO) Membership
Experience with clinical documentation in an Electronic Health Record
Canadian Nurses Association (CNA) and/or other related certifications
Membership in Specialty related Professional Association
