POSITION SUMMARY
In keeping with the mission statement of the hospital and the patient service units, the Care Facilitator is to provide clinical and practice support to the nursing staff.
This position will facilitate interdepartmental communication and will be instrumental to the management staff on call throughout the weekend for issues related to staffing and patient flow.
JOB DESCRIPTIONCo-ordination of patient movement as the need arises, facilitating, in a timely manner, the efficient functioning of the PSU with attention to assessing and directing care and flow of patients. Provide just-in-time clinical /practice support to nurses. Facilitate staff mobilization. Provide on-going clinical/technical education that is PSU specific. Responsible for policy dissemination. Fosters the provision of family-centered care by supporting and assisting nursing staff with patient care as deemed necessary and facilitating the advancement of evidence-based practices. Act as a resource person for unit staff, supporting and promoting efforts leading to the accomplishment of goals and individual/unit objectives. Assists staff members in carrying out the nursing process as appropriate. Works cooperatively with the Clinical Manager and the Clinical Educators to maintain continuity of patient care. Provides leadership in quality practice. Monitors and ensures that unit standards of care are maintained through direct observation of care, revision of charting and evaluation of equipment. Facilitates and initiates Quality Improvement activities and participates in unit quality management program.

QUALIFICATIONSVulnerable Sector Check (Essential) BScN (Preferred) Current registration with the College of Nurses of Ontario (Essential) Certification in one clinical pediatric specialty (Essential) Minimum five (5) years RN experience in a pediatric clinical environment, including a minimum of two years experience with pediatric triage, charge nurse training and a minimum of two years as a designated preceptor (Essential) Demonstrated leadership skills and abilities (Essential) Demonstrated ability to assign work, to monitor work requirements and to deploy resources appropriately and effectively (Essential) Demonstrated ability to assess critical situations quickly, to determine action needed and to make effective decisions (Essential) Demonstrated ability to diffuse situations which may be sensitive and/or highly intensive (Essential) Demonstrated ability in working with families during crisis situations (Essential) Must possess highly effective verbal and written communication skills (Essential) Must provide evidence of ongoing professional development and learning (Essential) Knowledge of adult learning principles and previous teaching experience/ability (Essential) Demonstrated excellent conflict management and interpersonal skills (Essential) Member of professional association(s) i.e. RNAO (Preferred) Bilingualism (English/French) (Preferred)

RESUME DES FONCTIONS
Conformement a l'enonce de mission de l'Hopital et des unites de services aux patients, le ou la titulaire fournit un soutien clinique et professionnel au personnel infirmier.
Il ou elle facilite la communication entre les services. Son role est crucial pour aider le personnel d'administration qui est de garde pendant les fins de semaine a resoudre des problemes de dotation et de deplacement des patients.
DESCRIPTION DU TRAVAILCoordonner la circulation des patients selon les besoins, faciliter en temps opportun le fonctionnement efficace de l'USP en evaluant et en dirigeant les soins aux patients et leur deplacement. Fournir en temps opportun du soutien clinique et professionnel aux infirmieres. Faciliter la mobilisation du personnel. Fournir de l'instruction clinique et technique continue qui concerne particulierement l'USP. Assumer la responsabilite de diffuser les politiques. Favoriser la prestation de soins centres sur la famille en soutenant et en aidant le personnel infirmier lorsque necessaire et en facilitant la promotion de pratiques fondees sur des donnees probantes. Agir a titre de personne-ressource pour le personnel de l'Unite, en soutenant et en promouvant les activites qui contribuent a l'atteinte des objectifs de l'Unite et de ses employes. Aider les membres du personnel a suivre les processus infirmiers lorsque necessaire. Cooperer avec la gestionnaire clinique et avec les formateurs cliniques afin d'assurer la continuite des soins aux patients. Diriger l'assurance de la qualite des pratiques. Veiller a ce qu'on respecte les normes de soins en observant directement les soins, en examinant les dossiers medicaux et en evaluant l'equipement. Faciliter et lancer les activites d'amelioration de la qualite et participer au programme de gestion de la qualite du Programme.

QUALIFICATIONSVerification des antecedents en vue d'un travail aupres de personnes vulnerables (Essentiel) B.Sc.Inf.
