The Behaviour Support Manager reports to the Director of Care and will serve as an in-house, on the identification and management of responsive behaviours. This dedicated Manager will function in a specialty role providing clinical care to older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions.

The Behaviour Support Manager will work in collaboration with the Behaviour Support Resource Team, Psychogeriatric Resource Consultants Geriatric Mental Health Outreach Team, Long Term Care Behaviour Support Outreach Team, hospitals, Home and Community Care Support Services, primary care providers, and other providers and professionals. The Behaviour Support Manager may also provide expert assistance
to neighbouring LTCHs.

Responsibilities:
\xe2\x97\x8f Provide sound clinical knowledge and skills, act as clinical expert and resource, and
provide comprehensive, quality client/family centered health care services.
\xe2\x97\x8f Collaborate with Psychogeriatric Resource consultants and mobile behaviour support
resources in the development/refresh of the LTCH\'s Behaviour Support Resource Team
(BSRT).
\xe2\x97\x8f Provide leadership and support to the LTC home\'s BSRT.
\xe2\x97\x8f Facilitates Quality initiative meetings to ensure appropriate use of medications in the
interest of resident safety and quality of life, lead the process to identify residents for
antipsychotic medication reductions and implements non-pharmacological and
monitoring strategies to manage expressive behaviours.
\xe2\x97\x8f Participate in interdisciplinary care conferences with residents/families and health care
team members for identification and management of the resident\'s personal expressions
as required.
\xe2\x97\x8f Coach LTC home staff in the use of frameworks such as PIECES and UFIRST to assess
responsive behaviours, identify triggers and develop behaviour care plans and approaches
to providing care to residents requiring behavioural support as well as their families.

\xe2\x97\x8f Provide comprehensive bio-psychosocial geriatric assessments, treatment, follow-up and
discharge planning in collaboration with the BSRT.
\xe2\x97\x8f Provide coaching on the implementation of behaviour care plans and guide appropriate
care plan transitions.
\xe2\x97\x8f Aid in the transition of long-term care residents with responsive behaviours (new or
existing) between community, hospital and neighbouring long-term care homes (LTCHs).
\xe2\x97\x8f Convene and chair regular Behaviour Rounds that enable front-line staff to discuss care
plans with internal and external resources.
\xe2\x97\x8f Identify the need for additional consultation and/or referrals to external services or
specialties and coordinate referrals.
\xe2\x97\x8f Advocate for and protect the individual rights of residents/clients.
\xe2\x97\x8f Raise client-specific challenges/barriers to the supervisor, as a means of identifying
systemic issues where broader consultations may be required.
\xe2\x97\x8f Proactively identify training and education needs for the home and coordination with the
PRCs to deliver capacity building support.
\xe2\x97\x8f Participate in the Orientation process of new employees, volunteers or students in regards
to the Responsive Behaviour program.
\xe2\x97\x8f Other duties as assigned and or indicated in Job Task Inventory

Qualifications:
The qualifications needed to join our family are as follows:
\xe2\x97\x8f Current registration with the appropriate College registered with a regulatory authority in
a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent
of a certificate of registration authorizing independent practice in Ontario as of right
exemption Bill 60,
\xe2\x97\x8f Current CPR/First Aid certification
\xe2\x97\x8f Training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Dementiability
are an asset
\xe2\x97\x8f Minimum one (1) year experience with clients in long-term care or residents in a long-
term care home setting
\xe2\x97\x8f Demonstrated knowledge and skill working with older adults presenting with or at risk
for responsive behaviours that may be associated with dementia, complex mental health,
substance use and/or other neurological conditions
\xe2\x97\x8f Demonstrated ability to coach and mentor others
\xe2\x97\x8f Demonstrated ability to take professional accountability for leading, organizing,
providing and delegating care activities for residents and families
\xe2\x97\x8f Experience with quality improvement methods and processes an asset

\xe2\x97\x8f Basic computer skills - Microsoft Word, Excel and online learning utilities including
ability to use electronic communication devices preferred
\xe2\x97\x8f Knowledge and understanding of RAI-MDS process
\xe2\x97\x8f Demonstrate flexibility in schedule to ensure all shifts (D,E,N) get support if needed
\xe2\x97\x8f To show evidence of being focused on people\'s feelings when interacting and knowing
how to turn a task into positive social interaction.
\xe2\x97\x8f To undertake and meet all legal requirements within a context of promoting individuals\'
rights and measuring/reducing elements of risk.
\xe2\x97\x8f Must provide a complete and current (within six months) Vulnerable Sector Check
(including a Criminal Background Check) or be willing to obtain one
\xe2\x97\x8f As a condition of employment, candidates are required to submit documented results of
TB testing (within the last six months or within 14 days upon hire) or a chest X-ray
(within the last year), as per Public Health requirements
\xe2\x97\x8f Two supervisory references required
\xe2\x97\x8f COVID-19 vaccination is a condition of employment
