The primary purpose of this position is to assist the Individual Planning Resource Development and Oversight (IPRDO) Director to provide oversight to Outside Paid Resources (OPR) and Third Party Staffing Agencies that support those adults who have not been able to transition to adult funded Developmental Services assigned to Community Living Fort Erie (CLFE) by the Ministry of Children, Community and Social Services (MCCSS). Ensuring dignity and respect to all people supported, based on values and outcomes important to each person on an individual basis.

STATEMENT OF MAJOR RESPONSIBILITES:



- The IPRDO Assistant Program Manager works under the general supervision of and is accountable to the IPRDO Director. Major decisions and those involving policies are referred to the IPRDO Director.

- Responsible for assisting the IPRDO Director to ensure that the OPR's and Third Party Staffing Agencies provide residential support services at a level in accordance with the current policies and procedures of the Ministry of Children, Community and Social Services (MCCSS) of Ontario. These policies and procedures shall reflect all Ontario Regulation 299/10 Quality Assurance Measures.

- Work in collaboration with the IPRDO Director to evaluate whether the adults brought forward to CLFE through MCCSS are appropriate for CLFE to provide brokerage and oversight.

- Work in collaboration with the IPRDO Director to review current placements to determine if they are suitable and if not explore and secure alternative living accommodations with a primary outcome of securing a placement with a MCCSS funded Transfer Payment Agency (TPA).

- Work in collaboration and assist the IPRDO Director to secure appropriate placements for CLFE Multi Year Supported Living Plan (MYSLP) funded adults according to their plan of care.

- Maintain a visit schedule with the placement in accordance with guidelines - at minimum, 7 days after intake, followed by once a month for the first 6 months and then every 90 days following

- Assist the IPRDO Director to develop and update yearly a Contract of Services Agreement between CLFE and the OPR outlining the responsibilities of the individual, family, OPR and CLFE.

- Assist the IPRDO Director to develop interim Contract of Service Agreements as required.

- Work in collaboration and to assist the IPRDO Director to work with existing Ministry funded TPAs to develop placements.

- Work in collaboration with IPRDO Director to consult and discuss needs, goals and direction with each individual, family and other professionals, evaluating progress and giving ongoing feedback as needed.

- Work in collaboration with the IPRDO Director to assist in monitoring and analyzing expenditures within allotted budgets for MYSLP funded placements.

- Assist the IPRDO Director to apply for additional funds as needed for change of circumstance with MYSLP funded adults.

- Work in collaboration with the IPRDO Director to coordinate with placement agencies in developing transition plans.

- Attend Plans of Care, and assist the IPRDO Director to monitor the residential placement, including the residential provider's implementation of the clinical plan.

- In collaboration with the community, updating the clinical plan as required.

- Responsible for ensuring that the support deemed necessary by each individual is met and maintained and that these supports encourage maximum independence and a valued social role.

- Continually evaluate the quality and effectiveness of all supports by putting into effect outcome based performance measures.

- Assist the IPRDO Director to prepare and present MYSLP funded individual profiles to RMG. Ensuring the information is accurate, up to date and working toward meeting the individual's personal goals and needs.

- Be knowledgeable and aware of the OPR's policy on health and safety.

- Complete compliance reviews and provide support to ensure measures are met/maintained.

- Assist when necessary to complete Health and Safety inspections of the OPR quarterly.

- Assist the IPRDO Director to coordinate with OPR providers and community based teams to plan for transition to TPA.

- Coordinate with DSO and provide regular updates.

- Assist the IPRDO Director to develop community crisis plans for individuals, coordinate with community based resources to mitigate crisis, make decisions in a crisis situations.

- Develop new community partnerships with OPR placements.

- Coordinate alternative housing opportunities when OPR placements breakdown

- Provide referrals to appropriate Ministry funded services.

- Receive mentorship from the IPRDO Director on how to write and develop MYSLP.

- Serious Occurrence reporting and follow-up to MCCSS. Report and update the IPRDO Director/Executive Director of all Serious Occurrences.

- Participate in regular Team Meetings, Committee Meetings, Strategic Planning, Special Project Planning and other ad hoc committee meetings.

- Perform other duties as assigned by the Executive Director/IPRDO Director

- An Assistant Manager shall ensure that a worker:

- Works in the manner and with the protective devices, measures and procedures required by OHS Act and regulations

- Uses or wears the equipment, protective devices or clothing that the worker's employer requires to be used or worn

- advise a worker of the existence of any potential or actual danger to the health and safety of the worker of which the supervisor is aware;

- Where so prescribed, provide a worker with written instructions as to the measures and procedures to be taken for protection of the worker

- Take every precaution reasonable in the circumstances for the protection of a worker

- Assistant Managers must be competent as defined by OHS e.g. "competent person" means a person who

- Is qualified because of knowledge, training and experience to organize the work and its performance,

- Is familiar with this Act and the regulations that apply to the work and

- Has knowledge of any potential or actual danger to health or safety in the workplace

KNOWLEDGE AND SKILLS:



Must have a strong belief in Community Living - Fort Erie philosophy and values, a strong understanding of Personal Outcome Measures and the agency's Human Rights Statement. Must have sound knowledge of agency policies, practices, health and safety requirements in residential settings and MCCSS requirements. Experience with dual diagnosis and familiar with children's group home licensing and Child Welfare system is considered an asset.

QUALIFICATION CRITERIA;



This knowledge and training is normally acquired through a two or three year Community College program such as the Developmental Service Worker, Social Service Worker or Child and Youth Worker with three to five years' experience working with adults with intellectual disabilities and one or more years of experience in a supervisory role.

Pay: $30.00-$36.00 per hour
