This leadership role focuses on mentorship, team development, and operational efficiency while addressing the increasingly complex needs of patients transitioning across the continuum of care. You will provide critical support to the Transition Coordinators, assist with workload balancing, and ensure adherence to high standards of care. With your collaborative approach, you will act as a liaison between the team and other stakeholders, enhancing communication and ensuring that patient needs are met effectively. Position Summary The AHN for Transition Services is a pivotal role designed to provide leadership, mentorship, and operational support to the UAH Transition Services Team. Working collaboratively with the Unit Manager, the AHN for TS helps manage day-to-day team operations, provides guidance on complex cases, and ensures the consistent application of processes and standards across the team. This role is integral in promoting team development, optimizing workflows, and addressing challenges as they arise, particularly in high-stakes situations such as complex patient discharges.Description:As an Assistant Head Nurse, you will be responsible for staff assignments, patient flow and acting as a role model, clinical resource and advocate for nursing staff. In addition to providing frontline nursing care, as required, you will take a leadership role in the promotion of quality patient care and the implementation of operational priorities. Your in-depth knowledge and skills, advanced judgment and clinical experience, will assist in providing solutions for complex health care issues at all levels with patients, interprofessional teams, administrators and policy makers. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:Minimum of three years of experience as a Transition Coordinator. A minimum of five years of facility-based nursing experience, including at least two years of experience within a community setting (home care, supportive living, or facility living). Familiarity with TC roles, scope of responsibility, and the ability to advocate for appropriate use of TC resources within interdisciplinary teams. Ability to consistently produce high-quality assessments and profiles that meet required standards. Strong written and verbal communication skills, with proven ability to maintain professional communication with colleagues, leadership, and patients/families. Demonstrated ability to support colleagues with problem-solving and skill development. Demonstrated leadership skills, including a desire to mentor, provide education, and assist with new staff orientation. Must be able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines. Must be able to tolerate long periods of sitting or standing as well as repetitive movements. Must be able to lift and carry 10-15 kg regularly. Must be able to participate in hands on care of patients (i.e. transfers, bariatric clients, bending, twisting, walking distances). Must provide references that can substantiate above qualifications. Must be able to complete N95 Fit testing and tolerate same. Preferred Qualifications:Minimum of two years of experience in an Assistant Head Nurse or Charge Nurse role. Familiarity with technology, including proficiency in Connect Care, Pathways, interRAI, e-People, and Microsoft Office applications (MS Teams, Word, PowerPoint, Excel). Familiarity with, and comfort in, providing support to a variety of portfolios across the site. Passion for providing education and mentoring within the team.
