BASIC FUNCTION: There are an estimated 949,000 Canadians living with stroke each year. Life after stroke brings many challenges for stroke survivors, their families, and caregivers. Many feel isolated and alone, not knowing how to get the care they need for their recovery, who to go to for support, and where to find information to help rebuild their lives. After Stroke is March of Dimes Canada\xe2\x80\x99s (MODC) personalized stroke recovery program that helps survivors, and their families navigate the path forward after a stroke. The After Stroke Coordinator plays a key role in providing direct support to those impacted by stroke across Atlantic Canada. This position reports to the Regional Director, Atlantic Canada. The After Stroke Coordinator provides stroke survivors, caregivers, and families with personalized support along the entire stroke recovery journey. In the role, the After Stroke Coordinator works directly with the stroke survivor or, caregiver and their family to establish outcome driven goals that will support their recovery in a way that is meaningful to them. A key responsibility in the role is to listen to the needs of the individual and develop a unique recovery plan that encompasses programs, services, and resources offered at March of Dimes Canada, as well as other opportunities that exist within their community. The After Stroke Coordinator is expected to work collaboratively with both internal and external stakeholders to provide the best possible support throughout the After Stroke pathway and help with re-integration back into the community. Internally, the After Stroke Coordinator will work with other mission delivery colleagues to establish the most impactful action plan that includes all that March of Dimes Canada has to offer. Externally, the After Stroke Coordinator will work with key likeminded partners and organizations to expand and enhance the types of services that can be made available to stroke survivors, their caregivers, and families. Roles and responsibilities: Program Delivery/Client Support
  • Engage directly(virtually) with stroke survivors, caregivers, and family members on an ongoing basis, to understand their unique recovery needs through the completion of a validated needs assessment and act as an information resource and support system.
  • Assist clients with prioritizing and setting SMART goals and develop personalized stroke recovery plans.
  • Act as an information resource and support system that clients feel comfortable and confident connecting with.
  • Provide system navigation that supports clients in achieving their goals.
  • Work collaboratively with the After Stroke team across Canada to ensure a client and family-centered approach is embedded in all services delivered.
  • Contribute to the overall success of After Stroke through participation in meetings, educational sessions, and working groups as required.
  • Work collaboratively with members of other business units across March of Dimes Canada.
  • Manage program resources effectively to meet program goals and objectives in a changing environment.
  • Skillfully organize and manage an evolving caseload of clients.
  • Contribute to the ongoing identification and maintenance of resources for stroke survivors and their caregivers using designated databases.
Stakeholder Engagement
  • Work collaboratively with other professionals and organizations involved in the care of stroke survivors, caregivers, and family to ensure the best possible support is provided at all stages of recovery.
  • Support and champion After Stroke program awareness within the stroke community.
  • Field external inquires as required regarding After Stroke.
  • Contribute to the development of tools and resources to support stakeholder engagement.
Training, Knowledge Development and Reporting
  • Ensure privacy and confidentiality protocols are followed.
  • Work with the After Stroke team to ensure that March of Dimes Canada policies, procedures, and processes are followed.
  • Identify and bring forward areas for improvement pertaining to program delivery, resources, and services offered through After Stroke.
  • Contribute to annual program plans, supporting the establishment of objectives and key performance indicators that align with the broader organizational goals.
  • Support the implementation and utilization of evaluation tools and standards that demonstrate program impact.
  • Utilize the Customer Relationship Management (CRM) system to capture client data that is clear and complete.
  • Identify personal learning and development opportunities related to the role and stay up to date with new developments and trends in knowledge, treatments and services related to stroke.
Qualifications and experience:
  • A minimum of 3-5 years working experience in a healthcare or related field
  • Post-secondary education in relevant field, and/or related equivalent experience
  • Experience working with the disability community, knowledge of disability issues and/or lived experience of disability or providing person centred support will be considered strong assets.
  • Working knowledge of stroke systems of care and stroke recovery as experienced by a stroke survivor or caregiver of a person with stroke.
  • Experience in the provision of virtual support to individuals and families.
  • Awareness of crisis management strategies and understand the importance of risk management protocols.
  • Proficient in Microsoft Office Suite of programs.
  • Experience in client relationship management systems, client record keeping and use of a database.
  • Excellent interpersonal communication skills.
  • Strong written and verbal skills with the ability to communicate effectively and efficiently.
  • Strong organizational skills, ability to prioritize multiple tasks and deadlines.
  • Works effectively with and demonstrates respect for others.
  • Some evening work may be required.
  • Bilingualism is an asset
