Position Summary

Granddaughters Home Care is seeking an experienced Licensed Practical Nurse (LPN) with specialized training in Advanced Foot Care. This role focuses on delivering professional foot care services, conducting client assessments for foot care and supporting ongoing foot care appointments. You will work closely with the Clinical Care Specialist and Care Experience Manager to ensure outstanding care delivery and client satisfaction, while also reporting directly to the CEO.

This position is part-time/casual only currently.



Key Responsibilities could include (may change over time):

Provide advanced foot care services to clients in their homes and community settings. Lead first visits with new clients to establish a positive, trust-based care relationship and foot care plan. Create and execute all follow-up appointments in our Alayacare scheduling system. Ensure adherence to healthcare protocols, infection control measures, and OH&S standards; escalate concerns as appropriate. Responsible for sterilization of all tools and equipment through GHC account. Collaborate closely with the Clinical Care Specialist and Care Experience Manager on care planning, quality assurance, and client experience improvement initiatives. Maintain accurate and timely documentation using our Alayacare documentation platform Support education and mentorship efforts related to foot care, personal care and health promotion for caregivers and clients. Maintain strict confidentiality and uphold the highest standards of professional practice.
Qualifications

Active and unrestricted Licensed Practical Nurse (LPN) license. Advanced Foot Care able to use FCN designation (recognized by CLPNA) Minimum 2 years' direct experience performing client foot care assessments and providing clinical field support. Proven leadership abilities and the capacity to work both independently and within a multidisciplinary team. Exceptional interpersonal and communication skills with a focus on client-centered care. In-depth understanding of healthcare regulations, best practices, and occupational health and safety standards. High attention to detail, strong organizational skills, and a proactive problem-solving attitude.
