Family and Individual Support Worker



Job Purpose:



The Family and Individual Support Worker works under two programs to support individuals. Following the goals of Community Living BC this position promotes physical, emotional, and mental well-being, encourage independence, and enhance quality of life of adults with disabilities. This role also strengthens the capacity of families to manage everyday challenges by providing individualized support, promoting positive parent-child relationships, and implementing services as outlined in the MCFD contract.



Duties and Responsibilities:



The following responsibilities of the Family and Individual Support Worker consist of, but are not limited to the following:

Build supportive and respectful relationships with individuals and families to encourage open communication and trust.

Provide individualized support services to parents and individuals (including those over the age of 19) based on specific, identified needs.

Promote positive parenting practices and encourage meaningful parent-child interactions.

Assist individuals in building life skills to support independent or supported living, including daily living activities like hygiene care, household management, and social engagement.

Develop, implement, and regularly review individualized service plans in collaboration with the individual, their family, and the CDC team.

Support individuals and families in identifying and accessing community-based programs, resources, and activities that promote integration and well-being.

Plan and organize parent education workshops or support sessions as needs arise.

Develop resource materials to assist families of children and youth with special needs.

Maintain accurate and confidential program records, service documentation, and statistical data in accordance with Centre and program funding requirements.

Participate in service planning meetings, case conferences, and staff meetings as required.

Communicate professionally and effectively, both verbally and in writing, with team members, healthcare providers, community agencies, and other stakeholders.

Complete critical incident reports in accordance with CLBC protocols.

Perform other related duties as assigned by the Program Manager, Supervisor, or Executive Director.

Engage in ongoing professional development through training, workshops, and other approved learning opportunities.

Write reports for funders, as needed.

Maintain appropriate records for evaluation, funders and organizational requirements

Carry out other duties as assigned by the Program manager or designate



Additional Requirements:



Criminal record check Valid driver's license and safe driving record Valid First Aid certificate Use of a safe vehicle is required. Flexibility to work variable hours including evenings and weekends.

Qualifications:



Undergraduate degree or diploma in a related field of education, social work, child and youth care, education, psychology, therapies preferred. Demonstrated working experience in organizing and supervising programs and services. * Relevant experience working with individuals and families.
