With a passion for care towards clients, families and team members, the Adult Day Support Program Assistant is responsible for providing outstanding care and support to clients, families and volunteers within their designated scope of practice. The ADP Assistant demonstrates responsiveness to the holistic needs of clients by assisting with activities of daily living, promoting choices, maintaining dignity and facilitating client independence.
What You Will Do:
Client Centered Care:
Promotes service excellence and is responsive to the clients' needs in accordance with resident care plans, policies and procedures.
Provides care and treatments that are routine and standardized as defined following assessment of the client.
Demonstrates initiative, group leadership skills and the ability to work independently to ensure the success and organization of programs, activities, and outings.
Ensures that any change in the appearance, condition, or behavior of clients is reported immediately.
Promotes and encourages physical independence.
Uses appropriate methods of approaching and responding to the client's emotional and psychosocial needs.
Prepares supplies when necessary.
Documentation
:
Accurate, comprehensive, and timely documentation of clients' participation and progress in programs.
Maintains an inventory of office and program supplies and ordering as designated.
Records and maintains clients' statistics daily and calculates monthly.
Documents a summary of clients' progress, involvement in groups, activities and individual care as requested.
Safety:
Contributes to a safe environment for clients, the team and self by understanding and adhering to safety guidelines and practices at all times.
Team Work
:
Cares for and actively participates and assists other employees to achieve and maintain a responsible atmosphere of client centered care.
Communicates with other team members and care staff regarding suggestions or proposed changes for client programming.
Communicates client treatment, care goals, and any concerns with appropriate resource person.
What You Bring:
Grade 12 Diploma
Certificate or Diploma in health related field preferred.
1-2 years' experience with geriatrics is preferred.
Able to work effectively in a physically and mentally demanding environment.
Excellent interpersonal skills and strong verbal and written communication skills.
