Job Summary
The Admission Discharge Manager is responsible and accountable for the overall coordinating of clinical activities related to patient flow within the Eastern Urban Zone. They will work with staff and managers with the goal of ensuring appropriate admission placement, efficient patient flow, and facilitation of complex discharges. The Admission Discharge Manager functions in a leadership capacity ensuring appropriateness of admission and length of stay using approved selection criteria. They liaise and direct the plan for patient flow with admitting, managers, patient care facilitators, charge nurses, physicians, allied health professionals, community liaison nurses, and support departments. They will, in collaboration with staff, managers and physicians, initiate a plan of action for appropriate discharge planning and for patients with complex discharge needs. The Admission Discharge Manager will also liaise with managers and staff in acute care, community supports, and long-term care to facilitate timely discharge in accordance with NL Health Services strategic plan. The Admission Discharge Manager will work with the health care team, management and physician leaders to resolve complex bed utilization issues to improve patient flow. The Admission Discharge Manager also participates in the planning, development, and implementation of initiatives to improve clinical efficiency. The admission discharge manager will work with counterparts in all NLHS Zones as well as out of province facilities to coordinate repatriation of patients.
Job Qualifications
Education
A bachelor\'s degree in a health related discipline required.
Master\'s degree considered an asset
Experience
A minimum of five (5) years of acute care clinical experience within the last seven (7) is required.
Previous management experience would be considered an asset.
Previous experience with data collection and information technology is required.
Change Management, LEAN, Emerging Leaders course considered an asset.
Competencies (Knowledge, Skills & Abilities)
Demonstrated knowledge of Clinical Efficiency Management and Discharge Planning principles are considered an asset.
Demonstrated excellent communication, negotiation and facilitation skills.
Demonstrated administration, leadership and conflict resolution skills.
Demonstrated time-management and organizational skills.
Other
Current registration/licensure in the relevant professional association is required.
A satisfactory record of work performance and attendance is required.
