Conduct intake assessments and develop individualized service plans for each resident
Provide individual and group counseling to residents, focusing on recovery support, life skills development, and goal setting
Connect residents to other social services or assist clients in locating and utilizing community resources including legal, medical and financial assistance, housing, employment, transportation, day care and other services
Facilitate life skills training and educational groups on topics such as budgeting, nutrition, and stress management
Collaborate with our multidisciplinary team to ensure a supportive and safe living environment
Maintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients' overall progress
Maintain accurate records and reports, including maintaining HIFIS, resident progress and program evaluations
Track and report on resident progress regularly.
Participate in staff meetings and training sessions.
Transport residents to appointments and activities
Ensure compliance with all organizational policies and procedures
Qualifications and Experience
Required:
Diploma in Social Work, Counseling, or a related field
2+ years of experience in a similar role, preferably in a substance abuse treatment setting
Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or equivalent
Strong knowledge of addiction treatment and recovery principles
Demonstrated excellent administrative and organizational skills, strong computer skills specifically in Microsoft Office Suite, including MS Word, PowerPoint, Excel, Outlook and TEAMS.
Excellent communication and interpersonal skills
Ability to work independently and as part of a team: 80% of position is independent work
Ability to maintain confidentiality and respect clients' privacy.
Lived experience with addiction recovery (preferably at least 2 years in sustained recovery).
Clean driving abstract G licence required.
CPR Certification or willingness to obtain
Police Vulnerable Sector check is required.
Preferred:
Experience Working with a non-profit or social service environment.
Experience with trauma-informed care.
Familiarity with local community resources and services.
Strong communication and active listening skills.
