Job Description

Reporting to the Chief Nursing Officer, the Acute, Long-Term and Transitional Care Manager is responsible for the provision of safe, high quality care to our residents and patients, while ensuring compliance with Ministry, legislative, and Hospital expectations.

Responsibilitie

Plan, direct, coordinate and supervise the care provided to patients and LTC residents. Support the development of individualized care plans with patients/residents, caregivers and team members. Identify strategies and resources to attain the desired clinical outcomes, and coordinate a safe and effective discharge for acute care patients. Work with the Chief Nursing Officer to hire, direct, train and evaluate nursing personnel. Review and develop policies, procedures, and standards of care required to provide quality care in accordance to best practices and the Long-Term Care Home Act and Regulations. Ensure safe, therapeutic, and ethical nursing care through the application of the quality best practices, nursing processes, and education. ? Act as a representative on internal and external committees, projects and/or working groups. ? Complete internal and external reporting requirements.

Qualifications, experience and knowledge

Current registration as a Registered Nurse with the College of Nurses of Ontario (BScN preferred). Experience in geriatric and/or general medicine nursing is preferred. At least 5 years of supervisory experience. Demonstrated leadership, communication and interpersonal skills. Knowledge of the Long Term Care Homes Act and Regulations, Public Hospitals Act, Occupational Health and Safety Act, and the Accreditation process. Knowledge of the nursing process, with advanced assessment skills and medical knowledge. Specialized education in health services management, gerontology or palliative care is an asset. Current C.P.R. Certificate. Bilingual (French and English)
