Canadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years.
During that time, it has evolved to provide a broad range of services to individuals and their families experiencing mental health challenges, from information and education to case management, psychosocial rehabilitation, housing and primary healthcare. These services are provided through inter-professional teams focused on providing clinically based services that, in addition to dealing with client mental health issues, address social determinants of health using the recovery model.
Overview
: Oversees and manages the day-to-day operations of the Assertive Community Treatment (ACT) Team and the step-down program (STEP) Team within the Canadian Mental Health Association (CMHA) Durham
.
Responsibilities
:
Organizes the day-to-day activities within ACT & STEP, managing a high performance multi-disciplinary team of staff, preparing work plans that align with the organization's goals and objectives
Directs staff in providing support, guidance and counselling in accordance with Ontario ACT Standards to facilitate medication management, psychiatric rehabilitation, activities of daily living, goal planning, clinical assessments, community resources etc.
Ensures that the day-to-day services are planned, delivered and evaluated in accordance with professional standards, good operating principles, best practices and legislative requirements
Directs and coordinates the client admission, treatment and discharge process
Participate in cross-functional teams, developing, recommending and implementing short and long term plans for the organization
Continually evaluates the status of clients and appropriately plans and coordinates treatment activities to ensure attention is given to their changing needs
Responsible for a documented staff management process including recruitment, establishing goals, strengthening competencies and supervision/evaluation of performance
Creates and maintains a positive work environment where all employees can maximize their job satisfaction and productivity
Ensures all accreditation and accountability requirements are met
Responsible for maintaining positive relationships with community partners and promoting/facilitating community service integration as required
Develops and coordinates all ACT & STEP policies, procedures and guidelines
Facilitates team meetings focusing on business and operational issues
Maintains up-to-date knowledge on ACT & STEP in a multi-disciplinary environment in order to ensure availability of service to clients
Prepares, maintains and monitors monthly and annual statistics as required
Oversees and audits for accuracy the maintenance of all ACT & STEP clients' documented information as per CMHA Durham policies, procedures and guidelines
Oversees staff coverage to meet client care needs
Ensures continuous improvement to enhance organizational strength
Oversees and manages departmental budgets and expenses to ensure appropriate funding allocation
Performs other tasks as assigned by management
Risk Management
Identifies, assesses, evaluates risk management activities and controls risks ensuring appropriate and sustainable activities/practices are developed and implemented
Anticipates risks to lessen exposure ensuring appropriate and adequate oversight is provided
Evaluates risk management practices ensuring ongoing, systematic process of identifying, assessing and prioritizing risks as they pertain to client information practices
Maintains membership with the Ontario ACT Association (OAA)
Skills:
Post-secondary education, minimum of 5 years clinical experience in mental health and a minimum of 5 years related management experience
A professional regulated under the Regulated Health Professions Act (e.g., nursing, psychology or occupational therapy) or is a registered social worker is preferred.
Sound knowledge of the Ontario Mental Health Act
Confidence when communicating with community service providers and professionals (i.e., probation officers, lawyers, landlords, police)
Good knowledge of psychiatric assessments in mental health especially in relation to current and future clinical programs and services
Extensive knowledge of client information management in mental health services
Proven ability to provide staff supervision and team leadership
Experience with methods of program evaluation
Proficient in crisis management intervention and recognition of unsafe environments
Ability to work with individuals of diverse backgrounds
Extensive knowledge of Regulated Health Professionals required documentation practices in mental health/health care services
Expert level administrative and organizational skills
Excellent communication skills (written and verbal) to ensure staff understand and contribute to the strategic vision of the organization
Proficiency in MS Office, documentation software and the internet
EFFORT
Sits at desk, stands and walks daily, and operates personal vehicle in order to perform the duties of the job
Concentration, conceptualization, attentiveness, creativity and innovation required daily using sight, hearing and touch to operate a computer, develop strategic actions, develop/provide information on policies and procedures, proofread, perform comprehensive assessments, write, respond to inquiries, read documents, resolve conflicts, problem solve, facilitate meetings, attend meetings, oversee daily operations, manage caseloads, prepare documents, listen to clients etc.
