We are looking for an Outreach & Housing Worker/Case Manager to join our Sources Housing Support Services team! This individual must be able to work independently, have a strong sense of social justice, and be able to work from a solution focused approach.

Description of Position
Reporting to the Housing Support Services Manager, the Outreach and Housing Worker position is a temporary employee of Sources Community Resources. This position ends March 31, 2026 and may be extended with renewal of the funding contract regarding this position. This position provides direct one-to-one comprehensive outreach and housing services for people currently undergoing the experiencing of becoming un-housed, released from hospital or living in the community with challenging health concerns that create risk of homelessness, or are currently homeless. The Outreach and Housing Worker provides support through crisis, working with clients to manage crisis, strategize solutions and next steps in accessing resources, services, shelter, housing and supports to assist the clients in achieving their housing stabilization plans. Addresses challenges with landlords or other services that may be contributing to the crisis. Provides written communications on behalf of the client; negotiates and mediates for the clients. Ensures access to all pertinent services and encourages development of support networks; promotes community relations, partnerships with project partners, and other human serving organizations and general public. Service includes providing support, information, referral, completion of forms and applications with Income Assistance and disability benefits. This position includes work with agency partners to assist with system navigation and providing services to individuals accessing Overdose Prevention Services and other resources. This position includes driving a society vehicle with clients in the vehicle, as needed.
This position includes work with agency partners and community partners in collaboration.
General Responsibilities
Provides outreach support to people in their crisis location & community.
Collaborates with the client in determining needs and identifying barriers the person faces to sustain or obtaining housing.
Provides assistance to the client in creating their housing and stabilization plan for reducing barriers to sustaining or obtaining housing.
Provide assistance with communications with landlords, navigating complex process, linking clients to other supportive networks and agencies such as mental health and health care services; provide ongoing contact and support in ensuring action.
Maintains complete and accurate statistics and records for all project activities. Meets all reporting requirements.
Drive clients to view housing and access other services.
Assist clients to access treatment, healthcare, Overdose Prevention Services, and other related services as needed.
Qualifications
Minimum 2 years post-secondary education and 2 years housing worker experience
Effectively utilize Office suite including Excel, Word, Outlook and central data systems
Effectively communicate professionally, verbally and written in English
Non-violent Crisis Intervention Training (Training may be accessed through Sources)
Clear criminal record check
Valid Class 5 BC Driver's License and clear driver's abstract (mandatory)
