AccessAbility Assessors provides knowledge, skill, and expertise in the delivery of care plans and coordination of appropriate interventions for individuals with disabilities and their families. The Assessor is primarily responsible for completing initial functional assessments and periodic review assessments for all individuals accessing the AAS and will analyze and interpret these assessment findings to determine funding and support services. Another primary responsibility of the Assessor is to support all social program staff in offering a person-centered planning approach related to case management based on needs vs. entitlement. The Assessor will collaborate with the program eligible individual, their family, their support network, and their AAS Case Worker to ensure that the case plan focuses on maximizing the client's opportunities for full citizenship. The Assessor will have significant personal interaction with the client and his/her family and will collaborate extensively with other support agencies, healthcare professionals and community organizations. This position provides leadership in the promotion and delivery of services, promotes collaborative and team building practices, and assists staff in providing care that utilizes a strength-based approach.


Duties will include but are not limited to:




Determining eligibility for AccessAbility Support funding and services for individuals with disabilities by conducting comprehensive assessments that accurately captures how an individual's disability affects everyday functionality; Using clinical judgement and critical thinking to assist in determining appropriate supports for individuals and their families; Reviewing, analyzing, and evaluating the assessment summary to determine program eligibility; Conducting and analyzing additional assessments for a variety of the social programs within the organization as required; these may include, but are not limited to, the Seniors Assessment Screening Tool, functional assessments for the Seniors Independence Initiative or for Residential Housing. Assisting to provide integrated case planning by using clinical judgement to advocate for clients if program options and service delivery are not meeting the un-met needs of the individual; Supporting the strategic direction and implementation of various initiatives that improve client outcomes; Ensuring a client centered approach is utilized when AAS Coordinators are developing care plans; Mentoring staff members on a regular basis by offering encouragement and coaching; Assisting in the delivery of the care plan and coordination of appropriate interventions as required; Enhancing workplace wellness and support positive workplace culture. Communicating effectively with clients and/or families, staff members, the general public, federal and provincial government, non-governmental organization's service providers, funding agencies. Developing working relationships with alternate community resources, support and funding providers such as Workers Compensation, Canada Pension Plan Disability, Private Health Insurance, Private Liability Insurance, health care providers, federal/provincial government programs, etc. by working collaboratively with program management and multidisciplinary team members to establish a positive workplace culture; Performing additional assessments and duties as required.

Minimum Qualifications:




A Bachelor's degree in a social science or health related profession is required. A current license or eligible for registration with a designated professional association or licensing body is required. Strong knowledge and understanding of community-based support and navigational principles. Valid driver's license. Strong interpersonal skills and experience working collaboratively with inter-professional and inter-departmental teams. Strong technical and computer skills; important for learning new programs and software and for using the Integrated Service Management (ISM) System, Power Point, Excel, MS Word, and other programs. A good previous work and attendance record. Acceptable criminal record check and vulnerable sector record check

Other Qualifications:




A Master's degree is considered an asset. Experience working with people with disabilities and their families is preferred. Experience in performing functional assessments and using various assessment tools is preferred. Experience in change management is an asset. Experience and knowledge of collaborative case management principles and practices is preferred. Expertise in complex health and social needs planning is considered an asset. Additional relevant education and experience will be considered an asset. Experience and knowledge of motivational interviewing is considered an asset. Must become proficient in motivational interviewing through on-site training and development within 2 years of employment.
