AccessAbility Assessors provides knowledge, skill, and expertise in the delivery of care plans and coordination of appropriate interventions for individuals with disabilities and their families. The Assessor is primarily responsible for completing initial functional assessments and periodic review assessments for all individuals accessing the AAS and will analyze and interpret these assessment findings to determine funding and support services. Another primary responsibility of the Assessor is to support all social program staff in offering a person-centered planning approach related to case management based on needs vs. entitlement. The Assessor will collaborate with the program eligible individual, their family, their support network, and their AAS Case Worker to ensure that the case plan focuses on maximizing the client's opportunities for full citizenship. The Assessor will have significant personal interaction with the client and his/her family and will collaborate extensively with other support agencies, healthcare professionals and community organizations. This position provides leadership in the promotion and delivery of services, promotes collaborative and team building practices, and assists staff in providing care that utilizes a strength-based approach.
