The Registered Nurse (RN) located at the Wellness Centre will provide care under the Terms & Conditions outlined by the Ministry of Health for Homelessness and Addiction Recovery Treatment (HART) Hub care providers.
1. Report concerns related to staffing levels and client acuity to the Director of Client Care
2. Assesses the health status of assigned clients on the unit as outlined in the Standards of Nursing Practice:
a) Completes a comprehensive client history from a variety of sources including but not limited to the client, family, health care providers and/or community agencies. Data is gathered through various means such as interviewing, observing, inspecting, auscultating and subsequently reported and documented according practice standards and policies to maintain an accurate and comprehensive account of the client care journey
b) Utilizes a variety of strategies to foster therapeutic relationships, communicate care concerns and maintain positive working relationships within the healthcare team and with the client and their family members/care providers
3. Works with the client and family/caregivers to formulate a plan of care that meets client centered goals and objectives by:
a) Completing a comprehensive client assessment/reassessment to Identify the clients existing and/ or potential problems
b) Formulating desired outcomes specific to the client problems
c) Ensuring that desired outcomes, mutually agreed upon by the client and nurse, using SMART goal. These are:
4. Advocates for clients to achieve optimal client outcomes
5. Utilizes time management skills to provide care to clients and delegates or directs other team members as appropriate
6. Works collaboratively with the Director and the healthcare team to problem solve, plan, implement and evaluate unit specific improvement strategies
Qualifications:
Current registration in good standing with the College of Nurses of Ontario
Registered or eligible with the College of Nurses of Ontario
Non-Violent Crisis Intervention certification preferred
Applied Suicide Intervention Skills Training certification preferred
Trauma-Informed Care training certification preferred
Satisfactory vulnerable person's sector criminal background check
