WOUND CARE NURSE (RN or RPN)


Temporary Full-Time (Up to 12 Months)



JOB SUMMARY



Reporting to the Director of Clinical Operations, the Wound Care Nurse is responsible for providing specialized wound care and support within the Lower Limb Preservation Strategy, under the clinical guidance of the Nurse Practitioner (NP). This role includes wound assessment, treatment, education, and coordination of care, particularly focused on the prevention, management, and healing of lower limb wounds, including diabetic foot ulcers, venous leg ulcers, and pressure injuries.

The Wound Care Nurse will work collaboratively with the NP and the interdisciplinary team to deliver high-quality care, guided by best practices in wound care and the Model of Health and Wellbeing. Outreach to marginalized or at-risk populations and support for prevention-focused strategies are key components of this role.

Lower Limb Preservation Strategy - Full-Time (40 hours/week, up to 12 months):



The Windsor/Essex Lower Limb Preservation Strategy is a community-focused initiative aimed at early identification and intervention for individuals at risk of lower limb complications. The Wound Care Nurse will play a key role in delivering front-line wound care services and education, contributing to the goal of reducing the incidence of limb loss across the region. The nurse will participate in outreach, clinical assessments, and interprofessional collaboration, supporting care plans developed by the NP.

EDUCATION AND TRAINING



Registered Nurse (RN) or Registered Practical Nurse (RPN), in good standing with the College of Nurses of Ontario - required

Current Basic Cardiac Life Support (BCLS) Certification

Advanced wound care certification (e.g., IIWCC, WCC, or equivalent) - strongly preferred

Completion of additional training in foot care or diabetic foot care is considered an asset

QUALIFICATIONS AND EXPERIENCE



Minimum 2 years of clinical experience in wound care - preferably in community or outpatient settings

Experience working with patients with complex chronic conditions, including diabetes, peripheral arterial disease, or venous insufficiency

Strong clinical skills in wound assessment, documentation, debridement, infection management, and advanced wound dressings Experience with patient education and health promotion Knowledge of and experience working with marginalized or vulnerable populations Familiarity with social determinants of health and their impact on healing and wellness Proficiency in the use of computers and various software applications, including Electronic Medical Records and Microsoft Office suite of programs. Demonstrated ability to work independently and as part of an interdisciplinary team Excellent communication, organizational, and critical thinking skills Current valid Class "G" Driver's License, and appropriate insurance and reliable transportation to support work travel across Windsor-Essex County, as required Bilingual (English/French) language skills are an asset

JOB DESCRIPTION



Perform comprehensive wound assessments, including measurement, staging, and documentation Collaborate with the NP to implement and adjust treatment plans based on wound progress, infection status, and patient comorbidities Apply advanced wound care techniques, including debridement (within scope), compression therapy, offloading, and dressing selection Provide education and support to patients and caregivers on wound care, prevention, foot care, and skin health Participate in outreach activities targeting at-risk individuals in shelters, long-term care homes, or community clinics Collaborate with physicians, NP, foot care specialists, dietitians, chiropodists, and community partners to ensure coordinated care Monitor and report on wound healing progress, complications, and patient adherence Participate in data collection and quality improvement initiatives related to wound care outcomes Document care and assessments in the Electronic Medical Record as per regulatory and organizational standards Support development and evaluation of wound care protocols and tools Contribute to community awareness and education sessions related to limb preservation and wound prevention Ensure infection control practices are adhered to at all times Provide mentorship or education to students and other staff, where appropriate Participate in team meetings and interdisciplinary case conferences

OTHER CONDITIONS



Must be available to work evenings and weekends as required Candidates successful in obtaining employment will be required to provide a Criminal Record Check with Vulnerable Sector Screening valid within six months prior to start date
Temporary Full-time (40 hours/ week, inclusive of a one-hour paid lunch).

Salary at top current Ontario Health funded rate (RN): $74,148

Salary at top current Ontario Health funded rate (RPN): $53,159
