The successful candidate must self-identify as a member of 2SLGBTQIA+ communities.prideHealth is a Primary Health Care initiative that works to improve access to health services which are safe, respectful, coordinated, comprehensive, and culturally appropriate for members of the 2SLGBTQIA+ community. prideHealth works in collaboration with other services and initiatives within Primary Health Care and portfolio partners across Nova Scotia Health Authority to offer programming and support.Reporting to the Health Services Manager, the prideHealth navigator demonstrates flexibility and innovation in supporting members of the 2SLGBTQIA+ communities in accessing services, programs, and/or resources within the health system and community that can support health enhancing behaviours. The prideHealth navigator provide guidance and support for clients, including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living.The prideHealth navigator is team focused and will collaborate with Primary Health Care staff, family physicians and primary care providers, and other health professionals and community groups to maintain awareness of existing supports within the community and surrounding areas through a community development, population health, and collaborative approach. The primary responsibilities of the prideHealth navigator are to work with individuals and groups to support members of the 2SLGBTQIA+ communities to connect with appropriate services within the health system and within the community. The prideHealth Navigator will develop close working relationships and extensive collaboration with a multitude of community members, partners, colleagues and portfolio partners including Nova Scotia Health Authority (Public Health, Community Mental Health, etc.), IWK, Family Practice community, government programs and agencies, community based and non-profit organizations, research/academic partners, and other stakeholders. The prideHealth Navigator also works collaboratively with team members and partners to develop, implement, and evaluate a community-based wellness navigation model that guides their work. Additionally, the prideHealth navigator will support coordination of aspects of the prideHealth program via application of knowledge, skills and professional judgment; collaborative approaches to quality improvement; assessment of community needs and assets; engagement of community members, partners, and stakeholders; capacity building; provision and facilitation of continuing educational opportunities; operational planning for service delivery; and short- and long term program evaluation. The incumbent is a member of an inter-professional team and collaborates with colleagues to determine goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities. Excellent interpersonal and written communication skills are required.About the Opportunity ContinuedThe prideHealth Navigator\'s activities are guided by PHC-DFP priority areas, multiple stakeholders, and by community needs and, therefore, requires a professional approach. They work according to the mission, vision, values and policies of Nova Scotia Health Authority and PHC-DFP to support the development and implementation of PHC-DFP programs, services and initiatives throughout the zone. This work requires recognition and application of best practice and application of evidenced informed decision making.About YouWe would love to hear from you if you have the following:

  • Bachelor\'s degree required from an accredited institution in one of the following health care disciplines: Occupational Therapy, Social Work, Registered Nurse, Recreation Therapy.
  • Registration with respective professional regulatory association required if applicable.
  • Minimum of 3 years experience including: clinical practice related to discipline; working in Primary Health Care setting/community based team/community setting and minimum one year recent mental health
  • Facilitation/education skills utilizing adult education, self management supports, and motivational enhancement principles (both individual and group settings)
  • Experience working with people living with a chronic condition(s)
  • Experience in partnership development, stakeholder collaboration, and community development
  • Understanding of population health, health promotion strategies, and behaviour change principles
  • In-depth knowledge of local health authority and community based services
  • Demonstrates person-centred/family-focused, diversity relevant approaches to are Well developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability and willingness to change
  • Demonstrates innovation in program planning, implementation and evaluation, and quality improvement
  • Computer skills (MS Office, email and internet)
  • Annual CPR-AED certification and annual WHMIS training and certification
  • Valid driver\'s license and daily access to a reliable car for travel for work
  • Competencies in other languages preferred; French an asset
