About the Opportunity

The Community Health Team (CHT) is a community-based health initiative that focuses on wellness group programming, wellness navigation and working with community partners. The CHT works in collaboration with other groups/organizations in the community to offer programming and support. CHT wellness programming includes risk factor management, healthy eating, physical activity, mental wellness and parenting.

Reporting to the Health Services Manager, the Wellness Facilitator demonstrates flexibility and innovation in supporting citizens of the communities that the CHT serves in accessing the services, programs, and/or resources that can support health enhancing behaviours. They provide guidance and support for clients, including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living.

The Wellness Facilitator is team focused and will collaborate with the CHT staff, primary care providers, other health care providers and community groups to maintain awareness of existing supports within the community and in the surrounding areas through a community development, population health and collaborative approach. This individual also works collaboratively with the team and partners to develop, implement and evaluate the community-based wellness programming model that guides their work. The incumbent is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities. Excellent interpersonal and written communication skills are required.

About You

We would love to hear from you if you have the following:

  • Bachelor\'s degree in Nutrition from an accredited institution is required
  • Licensure by Nova Scotia Dietetic Association required with completion of Dietetic Internship accredited by Dietitians of Canada
  • Minimum 1 year recent clinical experience including individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles, and the same level of experience (professional and/or volunteer) working with community groups/partners building trusting relationships through working together on projects/initiatives/partnerships that benefit the public.
  • Facilitation/education skills utilizing adult education, self-management supports, and motivational enhancement principles (both individual and group settings)
  • Experience working with people living with a chronic condition(s)
  • Understanding of population health, health promotion strategies, and behaviour change principles
  • Knowledge of local health and community based services
  • Demonstrates person-centered/family-focused, diversity relevant approaches to care
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability and willingness to change
  • Demonstrates innovation in program planning, implementation and evaluation, and quality improvement
  • Computer skills (MS Office, email and internet)
  • Current CPR-AED certification.
  • Valid driver\'s license and daily access to a reliable car for travel for work required. The incumbent will be required to travel to work in various community locations throughout the zone.
  • Competencies in other languages preferred; French an asset

Hours
  • Permanent, Full-time position; 75 hours bi-weekly
  • Travel to other sites throughout the greater Halifax Regional Municipality (HRM) is required.
  • Shifts may include evenings, weekends and holidays.
Compensation and Incentives

$32.78- $43.21 Hourly
