The Squamish Nation's Yuustway Health & Wellness Division is seeking a skilled and compassionate Wellness Coordinator / Social Worker to join our dynamic and interdisciplinary team within the Ka?numet Primary Care division. In this impactful role, you will work directly with community members to provide trauma-informed, culturally respectful, and strengths-based social work support. You will be a vital link between our clients, their families, community partners, and internal departments, offering clinical assessments, short-term counselling, outreach, and care coordination that centres the unique identity, language, and traditions of the Sk?wx?wu?7mesh Uxwumixw (Squamish Nation). You'll work within our integrated model of care to support wellness across the lifespan, especially for those navigating mental health, substance use, and social challenges. What You'll Do

Provide clinical assessment, case planning, short-term counselling, and culturally safe care coordination.
Deliver outreach services and support harder-to-reach community members with a creative, compassionate approach.
Collaborate with interdisciplinary teams, including Primary Care, Community Health, Home and Community Care, and allied health providers.
Offer suicide risk assessment, family assessments, and wellness plan development for mental health and substance use needs.
Advocate for clients navigating complex systems, including health, housing, and substance use treatment.
Participate in interdisciplinary case conferences and community health initiatives.
Maintain accurate and timely documentation using Electronic Medical Records.
Liaise with other Squamish Nation departments and external partners to ensure wraparound support.
Contribute to culturally relevant programming and community outreach events.
Remain current with best practices and uphold registration requirements.
What We're Looking For

A combination of education in Social Work (or a related discipline) from a recognized institution.
Current registration and good standing with the BC College of Social Workers.
Experience in social work and/or therapeutic counselling, particularly with concurrent disorders (mental health + substance use).
Experience supporting seniors and coordinating care with health professionals.
Strong understanding of trauma-informed, harm reduction, and strength-based approaches.
Excellent communication, collaboration, and crisis intervention skills.
Ability to navigate complex systems and support clients in accessing care and resources.
Experience working with Indigenous communities is a strong asset.
High emotional intelligence, cultural humility, and a commitment to community wellness.
Valid Class 5 driver's license and access to a reliable vehicle are preferred.

Salary:
$ 75,000 - $ 112,500
