Temporary Full-Time Wellness Coordinator

Full-Time - 8:00am - 4:30pm (Monday - Friday)



The Manor Village at London is looking for an amazing Temporary Wellness Coordinator. The Wellness Coordinator will operate as a Health & Wellness Ambassador. Their primary responsibility is to support the Director of Care in all aspects of the Wellness department which includes scheduling and implementing health assessments for prospective and current Residents and scheduling and executing new Resident orientation sessions with the goal of creating personalized wellness plans.

Other essential functions and responsibilities include, but are not limited to, overseeing and maintaining the Medication Management Program, the Physician Clinics, the Emergency Response System, assisting the Director of Care in providing direction and supervision to the Health and Wellness Team. The Wellness Coordinator must be a dedicated and caring professional who assesses the clinical status of each of our valued Residents and makes recommendations as to the appropriate level of care and services.

Requirements and Accountabilities



Community



Implement Manor Village infection prevention and control procedures/protocols Coordinate health services from a broad range of agencies and community resources such as foot clinics, hearing aid screenings, dentures, massage, reflexology and dieticians and all other health related activities to promote and facilitate the company Preserving Independence philosophy Monitor Manor Village's Green Card system Coordinate, run and/or assist with various clinics Prepare monthly wellness related newsletter submissions and calendar Arrange monthly education and health promotion sessions Maintain designated community bulletin boards with new health related information Investigate all Resident incidents and file associated reports

Resident Care



Prepare charts for new Residents Prepare for new Residents a personalized Wellness Welcome Package / Preserving Independence (including facilitating appointments with Manor Village Wellness 'Experts') Function in a case management role within a multidisciplinary environment Accurately assesses, communicates and documents Residents' status Visit Resident following an incident, return from hospital stay, on a scheduled visit and when/where required, administers new health assessments Visit Resident in hospital, rehab center, or other health care facility to complete health assessment prior to Residents return to the community (as needed) Measure and accurately records vital signs (temperatures, pulse, respiration, blood pressure, weight and height) where appropriate Implement intervention programs and evaluate the overall Wellness plan for all Residents Facilitate various Preserving Independence (TM) services and clinics Update and review "flag list" at weekly team meetings Advise and assist Residents with completion of medical forms (referrals, special transportation, home care services etc.) when applicable Document ongoing assessment, nursing diagnosis, planning, interventions and evaluation on Residents' file plus incidents or accident reports Educate new and existing Residents of all wellness services available through The Manor Village Arrange tray service and follow-up service for Resident(s) who are ill or have returned from hospital Observe and report any significant changes in Resident behavior and health to the Executive Director and family Collaborate with Executive Director in family conferences Addresses difficult Resident behavior Maintain the Goals of Care, Capsules of Life and/or 911 program in conjunction with the applicable medical services standards by keeping Resident medical data up to date, accurate and easily accessible to on-call staff in the event of an emergency

Prospective Residents



Meet prospective Residents upon their personalized visit (tour) to The Manor Village and explain the options available through the Wellness Center related to daily activities and the companies Preserving Independence philosophy Coordinate and conduct Wellness Planning Sessions for prospective Resident s In conjunction with Executive Director and Lifestyle Marketing Coordinator, review the acceptability of prospective Residents

Emergency Response



Respond to urgent and/or Emergency Call and Response System, requests assistance when necessary Initiate EMS calls when necessary Manage the operation of the 24-hour Emergency Call and Response System in partnership with third party control centers. Conduct monthly test (common areas) and sign-offs of the 24-hour Emergency Call and Response System Conduct annual test (suites) and sign-offs of the of the 24-hour Emergency Call and Response System Train new staff on the 24-hour Emergency Call and Response System Educate and instruct new Residents upon move in on the 24-hour Emergency Call and Response System and obtain required sign-offs

Relationships



Provide support and counseling for Resident s and families Visit all Residents regularly and systematically in order to develop a trusting rapport and helping relationship. Assist Residents and family members in determining appropriate level of health care services and support Maintain close relationship with physicians, pharmacists, home health care coordinators, home care and support workers and others to facilitate independence in activities of daily living

Miscellaneous



Perform other duties as assigned or as required Cover the receptionist's lunch hour as needed or as scheduled (coverage days and times may vary depending on the Community)

Qualifications



Current RN or LPN license RN designation (Required) 2 years geriatric care experience Knowledgeable about the senior living philosophy Understands nursing function within a senior living community Maintains current CPR and First Aid certification

Physical and Sensory Requirements



(with or without the aid of mechanical devices)

Ability to stand, walk, stoop, kneel, crouch or crawl for prolonged periods Use of hands to finger, handle or feel objects, tools or controls, with each hand/arm Medium Physical Demand Level: ability to regularly lift and/or move up to 9 kg / 20 pounds, and occasionally lift up to 23 kg/50 pounds Must be in good general health and demonstrate emotional stability Good visual acuity Mental acumen Good communication skills Must be exhibit our values in everything that they do Must have excellent emotional intelligence Ability to communicate professionally with staff members and Residents
