Summary



We are seeking a Program Coordinator to join our dynamic team at Affinity Health. This role is essential in overseeing various programs within our organization, ensuring that we deliver high-quality health care services to our clients. The Program Coordinator will work closely with staff and clients to facilitate effective communication and program implementation. This individual will play a pivotal role in fostering aging-in-place environments by building strong resident relationships, coordinating support services, and driving collaborative programming.

Community Engagement & Facilitation:

Proven ability to engage with diverse senior populations, facilitate resident-led initiatives, and promote participatory decision-making. Strong relationship-builder with a resident-first mindset.

Program Development & Coordination:

Skilled in organizing and executing social, recreational, and health-related programs that promote independence, well-being, and mutual support among residents.

Health & Social Services Navigation:

Knowledgeable in the health and social service systems, with the ability to assess needs, make appropriate referrals, and assist residents in navigating services.

Supportive Services Orientation:

Attentive to tenants' day-to-day needs -- from minor practical support (e.g., opening jars) to identifying more complex social or cognitive changes and connecting residents with the right care resources.

Partnership Collaboration:

Experience collaborating with external stakeholders, healthcare providers, service partners, and building management to ensure a seamless, integrated approach to aging in place.

Operational Leadership:

Comfortable supporting the selection and onboarding of new NORC sites, contributing to staff training, policy development, and supporting smooth daily operations in coordination with the broader team.

Monitoring & Evaluation:

Ability to collect, document, and report on tenant interactions and program outcomes while maintaining strict confidentiality in line with organizational policy.

Communication & Conflict Resolution:

Excellent interpersonal and communication skills, with a proactive approach to problem-solving and issue escalation when necessary.

Adaptability & Initiative:

Comfortable working independently within a team environment, flexible, and solution-oriented -- ready to support tenants in practical, hands-on ways as needed.

Requirements



Proven experience in project management within the healthcare sector. Education : Preferred to have college degree Strong administrative skills with attention to detail. Ability to lead teams effectively and foster a positive work culture. Experience in recruiting or training staff is an asset. Excellent communication skills for educating staff and engaging with clients. Bilingualism in English and French is preferred; additional languages are an asset. Background in social work, recreation, community/business development, gerontology, public health, or a related field. Preference will be given to candidates with previous building management experiecne managing community operations.
Experience working directly with older adults or vulnerable populations in housing, health, or community service settings.

Strong understanding of aging-in-place principles and person-centered support.

Familiarity with Ottawa's local health, housing, and social service networks is an asset.

Comfortable working in a residential building setting and adhering to health and safety standards, including the use of PPE when necessary.

Pay: $40,000.00-$65,000.00 per year
