About the Role:
We are seeking a dedicated and compassionate Visiting RPN interested in a Full Time Permanent Evening (Monday through Friday 3pm-11pm) opportunity to provide high-quality nursing care to clients in their homes within the Greater Hamilton and Burlington area. This role is ideal for an experienced nurse who values autonomy, flexibility, and the opportunity to build meaningful relationships with patients and families in a community setting while leveraging their diverse skillset.
Key Responsibilities:

• Hours of work are Monday through Friday 3pm-11pm
• Participate in on-call rotations as required (typically twice per month overnights)
• Conduct in-home assessments and deliver nursing care based on individualized care plans
• Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
• Monitor patient progress and update care plans accordingly
• Educate patients and caregivers on disease management, medication adherence, and health promotion.
• Collaborate with interdisciplinary teams and task delegation to ensure continuity of care

Maintain accurate and timely documentation using electronic health records

Qualifications
Qualifications:

• Registered Practical Nurse (RPN) with current CNO licensure in good standing
• Available and willing to work Monday through Friday evening shifts and on-call as necessary
• Minimum of 2 years of nursing experience, preferably in ER or as a Visiting Nurse
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Valid driver's license and access to a reliable vehicle with willingness to travel throughout the community (average of 75 visits Bi-Weekly)
• Proficiency with electronic charting systems
• LEAP Training, Palliative Certification, considered an asset
