About Perennial

Perennial delivers integrated primary care and wellness services directly within retirement residences, combining on-site and virtual care to meet seniors where they are. By proactively anticipating the needs of older adults, we improve quality of life, reduce unnecessary hospital visits, and ease the strain on the public healthcare system.

Today, we are active in over 29 retirement residences across Ontario, Saskatchewan, Alberta, and Manitoba, with continued plans for growth and expansion.

Our interdisciplinary team including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes.

About The Role

We are seeking a Virtual Weekend Nurse Practitioner (NP) to provide consistent virtual care coverage on Saturdays and Sundays across our network of clinics, starting December 2025 or January 2026.

This role is entirely virtual, offering flexibility while ensuring reliable access to care for seniors and staff in our partner retirement residences. A consistent weekend schedule will be established to maintain continuity for residents and allow you to plan your personal commitments with confidence.

Responsibilities

Provide virtual primary and urgent care coverage across Perennial's network of retirement residences. Conduct remote assessments, diagnoses, and treatment plans for seniors. Prescribe medications, order diagnostic tests, and develop individualized care plans. Manage urgent concerns from residents, families, and care staff during weekends. Collaborate virtually with physicians, other NPs, and allied health professionals to ensure continuity of care. Maintain accurate and timely documentation in electronic medical records (EMRs). Support clinical quality and best practices across our multi-province network.

Qualifications

Clinical Experience:

Minimum 2+ years as a practicing NP, ideally with experience in primary care, urgent care, geriatrics, or virtual care.

Education:

Master of Nursing (MN) or equivalent graduate degree with NP certification.

Licensure:

Current registration in good standing with the College of Nurses of Ontario (CNO) or equivalent provincial body.

Multi-Province Practice:

Preference for candidates already licensed in more than one province (Ontario, Alberta, Manitoba, Saskatchewan), or willingness to obtain additional registrations.

Skills:

Strong clinical decision-making, comfort with virtual care platforms, and excellent communication skills.

Availability:

Must be available to work both Saturdays and Sundays on a consistent weekly schedule.

Passion for Seniors' Health:

Commitment to preventative care and improving quality of life for older adults.
