Full time Wage: $51,890 per year to start



About the Role





As a Transitions Case Worker, you will play a pivotal role in supporting individuals transitioning from homelessness to stable housing. Working within a Housing First and recovery-oriented framework, you will:


Engage with clients to assess strengths, identify barriers, and develop strategies to achieve housing stability. Guide clients through housing intake and move-in processes, including arranging viewings and signing leases. Support clients in overcoming challenges such as obtaining ID, and acquiring funding. Create personalized case plans, provide life skills coaching, and connect clients to community resources. Work collaboratively with the Housing Transitions team to find appropriate long-term housing solutions. Maintain accurate case notes, daily stats, and other documentation. Address client crises and create safety plans as needed. For Housing Transitions Case Workers working in PBSH buildings, assist with building cleanliness, room inspections, and guest management.



About You





The ideal candidate will demonstrate empathy, professionalism, and the ability to work effectively with vulnerable populations. You bring:


A minimum 2-year diploma in a social science discipline (or progress toward it). 2+ years of housing casework experience, ideally with vulnerable populations. At least one year of experience working in the addictions field. Knowledge of community resources, social service programs, and housing systems within Calgary. Understanding of Housing First, harm reduction, and trauma-informed care philosophies. Strong crisis management, rapport-building, and advocacy skills. Proficiency in MS Office and strong organizational abilities. A non-judgmental approach, with a high degree of compassion and empathy.



Position Requirements

:


Proof of COVID-19 vaccination. Class 5 (non-GDL) driver's license and willingness to drive agency vehicles. Physical ability to assist clients and lift items up to 35 lbs. A valid Vulnerable Sector Search (within the last six months). Certifications such as FOIP and "It's Your Move" (or equivalent Patient Lift training).
