Transitional Case Manager, Court and Justice Programs Full-Time, Contract (approximately 9 months) London, Ontario - Application Deadline: March 2, 2023 -
Working within a multi-disciplinary team framework the Transitional Case Manager performs duties and responsibilities consistent with providing a psychosocial environment to assist with the rehabilitation and community integration of individuals involved in the criminal justice system, including those with serious and persistent mental health concerns, developmental disorders, acquired brain injury and/or concurrent disorders.

WHAT WE\'RE OFFERING:

  • What you\'ll be making - $28.34-$31.85 per hour, rate based on proof of comparable, previous experience; mileage between sites reimbursed per kilometre
  • When you\'ll work - 70 hours over two-week period, Monday-Friday 8:30am \xe2\x80\x93 4:30pm; Occasionally you may be required to work outside of these hour
  • What your benefits will be - After a 90-day waiting period, eligible for a partial (20%) prescription drug benefit
  • Your pension plan - Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:
Assessment -In collaboration with the individual:
  • Complete intake/screener to support referral to Adult Therapeutic Court
  • Gather health related documentation as required for Adult Therapeutic Court
  • Conduct assessments, treatment planning, and reporting for the Mental Health Diversion Program
  • Provide information and referral support to individuals, family members, and service providers
  • High level assessment skills to assess clients\xe2\x80\x99 mental, physical and emotional functional abilities and needs.
Individualized Planning - utilizing the information obtained through the assessment process and in collaboration with the individual:
  • Assist the individual to identify personal goals and develop support plans that identify individual strengths and areas for personal growth and goals to meet determined needs.
  • Provide 1:1 support services to a caseload of individuals with mental health concerns who are currently involved in the criminal justice system
  • Provide skills teaching and support to clients to assist in goal achievement
  • Provide assistance with financial management, vocational and occupational support, and discharge planning.
  • Provide crisis support and crisis prevention planning
  • Engage in group facilitation as required that supports clients in completing their established goals for court diversion
Service Coordination and Advocacy:
  • Have knowledge of specialized courts
  • Have knowledge of community resources relevant to individuals, including other mental health and addiction services, entitlement and benefit programs, (e.g. ODSP, Ontario Works, etc.)
  • Attend court sessions, as needed, and provide court support to individuals dealing with current or past charges
  • Liaise with legal professionals and service providers to ensure a coordinated approach to service provision
  • Advocate for client rights as necessary and support self-advocacy
  • Identify, develop and maintain good links with a wide range of community resources
  • Work collaboratively with other service providers to ensure a well-coordinated approach to provide support and/or discharge planning from detention centres/forensic hospital, etc.

WHAT YOU\'LL NEED TO APPLY:
Vaccination Requirements CMHA Thames Valley Addiction & Mental Health Services requires that all staff members be fully vaccinated against COVID-19, with the rare exemption of those individuals who cannot be vaccinated due to approved exemptions (medical reasons or religious grounds as recognized in the Ontario Human Rights Code). Proof of full vaccination or approval of exemption will be required; further details will be provided to those candidates moving forward in the recruitment process. Police Vulnerable Sector Check Requirements Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
We require the following qualifications:
  • Registration with a relevant professional college is required (currently recognized colleges include OCSWSSW, CNO, CRPO, CPSO, COTO, CPO)
  • Valid Driver\xe2\x80\x99s Licence, adequate insurance and access to a vehicle to carry out job duties
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:
  • 3-5 years\xe2\x80\x99 experience working in mental health, preferred
  • Current CPR and First Aid Certification
  • ASIST Certificate
  • Experience working within the criminal justice system
  • Experience and detailed knowledge of serious mental health concerns, acquired brain injury, developmental disabilities
  • Experience working in a community-based setting
  • Working knowledge of a variety of treatment, interventions, and support strategies
  • Experience in crisis intervention/management and crisis resolution
  • Extensive knowledge of services in the Thames Valley region
  • Knowledge of the legal system
  • Planning and organizational abilities
  • Excellent communication skills
  • Proven ability to work in collaboration with others
  • Strong active listening and verbal communication skills
  • Computer skills
