Your Opportunity:As the Transition Coordinator you would be a member of the diabetes multi-disciplinary team and play an integral role in supporting adolescent patients ages 15 to 18 in successful transition from pediatric to adult diabetes care. You will have the opportunity to utilize excellent communication, teamwork, and relationship building skills to support the optimal outcomes for youths with diabetes as they transition into the world of adult diabetes care. You will be expected to contribute to clinical process improvement, quality improvement initiatives, reporting to and working with the medical teams, collaboration inn a positive and supportive working environment, and clear demonstration of accountability to the duties and responsibilities of this role. The role of the Transition Coordinator is highly complex and includes comprehensive assessment, planning and coordination of care, as well as ongoing communication with other health-care providers across the continuum of care. Upon hire, you will receive specialized training providing you the knowledge, tools, and resources to confidently care for these patients. Major responsibilities include responsibility for facilitating medical follow-up appointments and referrals, answering questions, and encouraging engagement youths with diabetes in their transition of care. This role will also facilitate group transition classes in support of youth education and engagement in transition of care. Required Qualifications:Bachelor\'s degree in a health-related field or bachelors of arts in communication. Advanced written communication skills (educational and professional documents) and proficient both verbally and written in English. Additional Required Qualifications:Aptitude for building and sustaining complex relationships with key internal and external stakeholders. Ability to use skills such as team building, negotiation and conflict management, and group facilitation to build partnerships. Anticipates potential challenges and comes up with solutions. Expert facilitation skills. Ability to manage and prioritize work and meet deadlines. Ability to work independently, set priorities and work simultaneously on a variety of projects and manage competing demands. Preferred Qualifications:Combination of education and work experience equivalent may be considered. Experience as a volunteer or employment with youths (e.g. camp counsellor, youth day programs, youth leader). Experience with research and project management methodology and change management, SharePoint and Microsoft programs.
