About the Opportunity

Reporting to the Provincial Manager, Mental Health and Addictions Intake, this position is responsible for the facilitation of client transfers in collaboration with zonal services of Mental Health and Addictions (MHA), coordinates all aspects of transitions of care into and between services of the Mental Health and Addictions Program. They facilitate all TOC processes to ensure clients/patients are connected to the most appropriate service and programs to meet their needs. The clinician establishes and maintains positive relationships with clients, families, transfer/referral sources and designated services, responding to requests and concerns on a timely basis.

The TOC Clinician also participates in quality initiatives in the program of care, engages in collaborative person-centered care, and utilizes available information technology to support best practice. It is an expectation that the TOC Clinician will act in a professional and ethical manner, following all recognized codes of ethics, standards, legislation, and guidelines relevant to clinical practice and profession. They will convey respect and a solid understanding of subtle and complex diversity and inclusion issues as they relate to specific groups.

About You

  • Bachelor\'s degree in a health-related discipline
  • Must belong to provincial (Nova Scotia) self-regulated health college/governing body. and be a member in good standing.
  • 4 years of clinical experience in MHA and be familiar with both inpatient and community settings
  • Experience working in an inter-disciplinary team structure
  • Flexibility in problem-solving and tolerance for the challenges inherent in working with MHA clients including those who have a chronic clinical presentation.
  • Demonstrated ability to manage crisis situations in a confident, proactive, and collaborative manner
  • Demonstrable leadership style developing and/or in place, such they can confidently and diplomatically inspire the confidence and support of colleagues
  • Demonstrated capacity to work independently and as part of a team
  • Demonstrated interest in ongoing professional and clinical development
  • Familiarity with navigating the information technology systems of a large health organization
  • Knowledge of DSM V
  • Knowledge and experience with a recovery-oriented approach /psychosocial rehabilitation
  • Demonstrated understanding of the Involuntary Psychiatric Treatment Act (IPTA)
  • Excellent knowledge of medications
  • Excellent understanding of medical terminology
  • Cultural awareness and sensitivity
  • Non-Violence Crisis Intervention (NVCI) preferred
  • Suicide Risk Assessment and Mental Status Assessment required
  • Proficiency with computer applications including Microsoft Word, PowerPoint, Excel, Internet, Outlook e-mail, and ability to use health information systems for decision support
  • Valid Nova Scotia\'s driver\'s license and able to travel to various Central Zone sites as needed

  • Permanent Full Time position; 75 Hours Bi Weekly
Compensation and Incentives

$33.80 - $42.68 hourly
