About the Opportunity
Reporting to the Provincial Manager, Mental Health and Addictions Intake, this position is responsible for the facilitation of client transfers in collaboration with zonal services of Mental Health and Addictions (MHA), coordinates all aspects of transitions of care into and between services of the Mental Health and Addictions Program. They facilitate all TOC processes to ensure clients/patients are connected to the most appropriate service and programs to meet their needs. The clinician establishes and maintains positive relationships with clients, families, transfer/referral sources and designated services, responding to requests and concerns on a timely basis.
The TOC Clinician also participates in quality initiatives in the program of care, engages in collaborative person-centered care, and utilizes available information technology to support best practice. It is an expectation that the TOC Clinician will act in a professional and ethical manner, following all recognized codes of ethics, standards, legislation, and guidelines relevant to clinical practice and profession. They will convey respect and a solid understanding of subtle and complex diversity and inclusion issues as they relate to specific groups.
