This is a Temporary Full-Time Opportunity
Type: New
Duration: Up Until December 31, 2026

Summary of Duties, but not limited to:
Performs an intake function for the Reproductive Life Stages Program, including:

  • Conduct telephone assessments which involve a) completing an intake questionnaire highlighting demographic information, reason for referral, psychiatric and medical history, medications and family history of mental illness; and b) administering psychometric depression scales (such as the Edinburgh Postnatal Depression Scale)
  • Providing recommendations regarding psychoeducational and community mental health resources
  • Liaises with referring providers and other clinicians within the Circle of Care to obtain collateral information
  • Triage patients using triage tools and resources
  • Identify individuals at risk for suicidality or harm to others and initiate emergency services (including 911 and Children's Aid Society)
Supports the Day Treatment Program by co-facilitating daily group sessions, including:
  • Daily check-in sessions
  • Weekly walk
  • Stress management sessions
  • Unstructured socialization session (i.e. using board games to develop social skills)
  • Assist the therapist in goal setting with clients
Supports the Trauma Therapy Program, including:
  • Triage referrals that are beyond the scope of the medical secretary
  • Conduct monthly orientation sessions
  • Co-facilitates a Stage One, Psychoeducational Group (Resourced and Resilient), including doing the individual assessments to determine eligibility
  • Liaises with referring providers and other clinicians within the Circle of Care to obtain collateral information
  • Provides discharge planning and recommendations regarding psychoeducational and community mental health resources
  • Respond to client inquiries related to their status on the wait list or their referral
  • Continue to build trauma-related resources for TTP clients Offers Supportive Counselling and education

  • Provides short-term supportive counselling related to housing, employment, financial stress or other stressors associated with acute life adjustment
  • Provides education related to the benefits of self-care, sleep hygiene, etc.
  • Identify individuals at risk for suicidality or harm to others and initiate emergency services (including 911 and Children's Aid Society)
Provides individual case management and discharge planning:
  • Working collaboratively with clinicians to identify and link patients with the appropriate community resources
  • Providing recommendations regarding psychoeducational and community mental health/trauma resources
  • Obtaining required collateral information (including reports and phone correspondence) from physicians and other clinicians to assist in further assessment
  • Participate in group supervision to collaborate with clinical team related to the coordination of care Other Duties

Qualifications
  • Registration with the Ontario College of Social Workers and Social Service Workers is required
  • Master's degree in social work is required
  • Minimum of 5 years' experience in a hospital, ambulatory setting is required
  • Experience working with clients in a mental health setting & with survivors of trauma
  • Education and experienced working with survivors of trauma from a trauma informed, client-centered/anti-oppressive framework.
  • Experience with non-violent crisis intervention and de-escalation
  • Strong conflict resolution skills
  • Experience working with an inter-professional team
  • Strong knowledge of community resources
  • Group work experience is an asset
  • Demonstrated continued professional development
  • Highly self-directed, organized and assertive
  • Able to manage demanding interpersonal situations and respond with good judgment and understanding
  • Ability to prioritize situations and manage time effectively
  • Good work and attendance record required
