JOB DESCRIPTION - FOSTER PARENT/HOST PARENT/LIVE IN MENTOR



The following information identifies the skills and experience required to be successful as a FOSTER PARENT/HOST PARENT/LIVE IN MENTOR.



Attend orientation to the Agency's Policy and Procedure Manual - sign acknowledgement forms that indicate understanding of specific Polies and Procedures.

Ability to develop and implement programs within the Agency's standards and service provision requirements that benefit the Service Users within the Agency.

Experience working within a set budget within the assigned location

Possess certification in the behavior management program that the Agency chooses to use (UMAB)

Possess certification in Standard First Aid and CPR

Possess certification in PRIDE - Pre-Service CAS Training, Trauma Informed, FNIM and Kujali training as per MCCSS

Willing to participate in professional development as offered by the Agency and to provide feedback regarding the knowledge gained through professional development

Ability to utilize the On Call Emergency system within the agency

Ability to communicate and facilitate on behalf of Service Users within the Community including school system and justice system

Be aware of the requirements under the C.Y.F.S.A and QAM from the Ministry of Children, Community and Social Services

Familiarity working with individuals with a dual diagnosis (developmental disability and mental health diagnosis)

Successfully complete a Vulnerable Sector Check and Medical Screening

Provide a safe, supportive, stable, nurturing and warm family environment.

Be patient, committed and flexible

Manage monthly program budgets including the completion of documentation

Prepare well-balanced and home cooked meals and snacks according to the Canada Food Guide

Preparing progress reports; presenting information regarding those progress reports; participate in group meetings, daily reports, incident reports etc.

Plan and implement life skills and recreational activities at least twice per week and complete documentation

Arrange for medical, dental, hearing, vision and other appointments for the Service User's ensuring transportation and attendance at all appointments.

Assist the Service User in understanding their rights and responsibilities within the Agency.

Where appropriate will design and implement short term treatment goals in consultation with professionals involved in the support of the Service User.

Assist in the development of an Individual Support Plan under the standards through Quality Assurance Measures.

Prepare accurate documentation as required by Bridging The Gap Children and Youth Services Inc., as required by the Ministry of Children Community and Social Services and

All other assignments as directed by Management as deemed appropriate

Ability to plan and implement activities within the shift at the assigned location

Knowledge and understanding of creating and implementing life skills programs

Ability to communicate clearly with all parties involved with the Service User, within the location; agency and community.

Have knowledge of abnormal behaviors, interpret the function of those behaviors and implement behavior supports as identified by the management team and/or professionals.

Ability to problem solve and de-escalate in crisis situations within the service principles of the Agency

Provide leadership and guidance to junior staff as well as Service Users within the location.

Demonstrate leadership abilities

Always demonstrate professional standards within the location including appropriate dress and appropriate communication.

Valid UMAB

Valid First Aid and CPR

PRIDE Pre-Service CAS Training

Trauma Informed Certification

First Nation Inuit and Metis Certification

Kujali - Caring for Black Youth Certification

Child and Youth Worker Degree/Diploma

Social Service Worker Degree/Diploma

Developmental Service Degree/Diploma

Experience in residential services and child welfare of at least 1 year

Job Types: Full-time, Freelance

Pay: $80.00-$150.00 per day
