Job Overview:
Reporting to the School Principal and the Mental Health Lead, the Social Worker works as part of the multidisciplinary team to provide advocacy, mentorship, assessment, recommendations, and support implementation plans to provide service and support to students and their families. This position will liaise with, and refer students to appropriate community agencies and provide brief clinical crisis intervention services.
Key Responsibilities:
Investigate and assess socio/emotional, mental health and/or behaviour needs of students referred through in-school teams.
Collaborate and consult with administration and in school teams to provide service to students and families.
Complete assessments, including case plans, provide support for plan implementation, and communicate results and recommendations to the system or in-school team for individuals or groups.
Provide support to students with mild to moderate mental health and addictions, and youth whose clinical presentation is appropriate for service within a school setting.
Offer brief clinical crisis intervention and support to students, staff, and families.
Work and liaise with system leaders, in school teams, and community partners to develop service plans and/or programs to meet the needs of students.
Make referrals to appropriate community agencies or medical clinics which will best serve the needs of the students and engage in case management follow up plans.
Qualifications:
Minimum post-secondary completion of a Bachelor of Social Work, and registration with the Ontario College of Social Workers and Social Services Workers in good standing.
Minimum of one (1) year of professional experience preferably in a community or educational setting.
Evidence of extensive knowledge and experience with theories, principles and practices of social work.
Knowledge and understanding with the ability to apply relevant regulations, acts, and policies such as Regulated Health Professions Act, Health Care Consent Act, Education Act, Child Welfare Act, Social Work and Social Service Work Act and MFIPPA.
Experience working in a multi-disciplinary setting is preferred.
Demonstrated experience with providing intervention strategies appropriate for children and youth.
Evidence of strong analytical, problem solving and communication skills.
Familiarity with mental health promotion, prevention and intervention services within a tiered system.
Knowledge and training in ASIST or other suicide intervention strategies, and crisis intervention support.
Knowledge of local community resources and services.
Satisfactory Vulnerable Sector Background check.
