This role is focused on enhancing awareness and access to culturally responsive, community-based maternal health services for underserved and vulnerable populations. The MaternalMobile Health Community Outreach Worker play a critical role in promoting maternal wellness, supporting prenatal and postnatal care, bridging gap and improving access to services at Hamilton Urban Core, and linking families to essential health and social supports.
Accountability:
The Maternal Mobile Health Community Outreach Worker is an integral member of the Hamilton Urban Core team and reports to the Senior Manager, Integrated Care and Client Experience, with overall accountability to the Executive Director.
Key Responsibilities:
Conduct targeted outreach to expectant and new mothers in marginalized communities through home visits, mobile clinics, community centres and events, encampments, shelters, and pop-up health services.
Build trust and provide culturally appropriate education on prenatal/postnatal health, nutrition, breastfeeding, infant care, and family planning.
Collaborate with healthcare providers and interprofessional teams to assess needs and deliver holistic, wrap-around maternal health services.
Advocate for clients in accessing health care.
Distribute educational resources and essential items such as prenatal vitamins, hygiene kits, and baby care supplies.
Support the planning and delivery of culturally relevant prenatal and postnatal workshops, peer groups, and health promotion activities.
Connect clients to appropriate services at Hamilton Urban Core and the community.
Act as a trusted liaison between clients and health/social service providers to reduce barriers and improve continuity of care.
Monitor and report on emerging community needs, trends, and systemic barriers affecting maternal health access.
Maintain accurate, confidential, and timely documentation in accordance with privacy regulations, organizational policies, and professional standards.
Engage in ongoing training and professional development to remain current on best practices in maternal health, health equity, and community-based care.
Apply anti-racism, anti-oppression, culturally sensitive, and language-appropriate frameworks in all aspects of client care and program delivery.
Work as part of the Centre's interdisciplinary team and adhere to Hamilton Urban Core's core competencies and the Community Health Centre Model of Care.
Actively participate in Centre-wide activities such as the Annual General Meeting, client centered events, advocacy efforts, and special initiatives related to Hamilton Urban Core's mission.
Adhere to all organizational policies, professional standards, and ethical guidelines in all aspects of client care and program documentation.

Qualifications:
Post-secondary education in Community Health, Social Services, Public Health, Nursing, Community Development, or a related discipline.
Lived experience as a member of one of the priority populations a strong asset.
Demonstrated experience working in maternal and child health, community outreach, or frontline service provision within diverse or underserved populations.
Solid understanding of health equity, social determinants of health, and trauma-informed practices.
Strong communication, relationship-building, and advocacy skills.
Experience working in multicultural, inner-city, or newcomer communities.
Fluency in a second language and/or lived experience in a racialized or newcomer community is considered an asset.
Valid Ontario driver's license and access to reliable transportation is preferred.
Current First Aid/CPR certification and a clear Vulnerable Sector Police Check are required.
