Overview
The Willows Community Care is seeking an experienced and dependable Care Manager to oversee daily care operations and ensure the needs of approximately 50 residents are consistently met. This role is responsible for coordinating resident care, supervising the care team, and ensuring all care is delivered in line with Care Home Standards and organizational policies.
This position requires flexibility, as it is not always a straight 9-5 role. The Care Manager must be available to respond to operational or care-related matters as they arise.
The ideal candidate will have strong experience in senior or community care, be confident leading staff, and be highly computer literate.
Key Responsibilities
Lead and supervise the care team to ensure safe, compassionate, and consistent care
Ensure the daily care needs of 50 residents are met and documented appropriately
Develop, implement, and monitor individualized care plans, including for residents with dementia
Monitor resident wellbeing and adjust care plans as needed
Ensure compliance with Care Home Standards, health regulations, and quality-of-care requirements
Oversee medication administration and ensure accurate documentation
Ensure staff complete proper progress notes and care documentation
Coordinate schedules and daily staffing needs
Provide staff training, coaching, and performance feedback
Track and administer required monthly education and training
Communicate and collaborate with physicians, nurses, social workers, families, and other healthcare professionals
Complete other duties as assigned to support operations
Required Experience & Skills
Proven experience in senior care or community care, preferably in PEI
Previous experience supervising or managing staff in a healthcare setting
Strong understanding of Care Home Standards and regulatory requirements
Excellent computer literacy, including confidence and efficiency using Microsoft products
Strong knowledge of care documentation
Organized, reliable and able to manage multiple priorities

This position does not require a specific designation, but does require healthcare experience, preferably in Community Care. This role offers the opportunity to make a meaningful impact through hands-on leadership and high-quality care for seniors. This is a temporary role covering the Care Manager for a 6 month period.

Pay: $21.00-$22.50 per hour
Expected hours: 40 per week
