Job Description

The St. Joseph's Health Centre Palliative Care program includes a state-of-the-art 10-bed PCU and the outpatient Palliative Care and Symptom Improvement clinic (PACSI). These services combine to provide holistic and integrated person-centered care across the continuum of palliative services. Given the highly integrated nature of our inpatient and outpatient services, our program is seeking a highly organized and quality focused individual who will function as the team leader for the PCU and PACSI Clinic.


The PCU and PACSI clinic TL will work in a dynamic and evolving environment, collaborating with inter-disciplinary teams to provide safe, effective, ethical and compassionate care for patients and their families. This role will also support the development of team members as they learn and apply palliative specific skills when caring for and interacting with patients and their families. The successful candidate will need to have experience providing comfort focused care, engaging in difficult conversations and expert bedside nursing skills in order to manage palliative crises requiring intervention.

Duties:

Manage nursing assignments Support PCU nurses with workload and bedside care as operationally required Triage support for the PCU including external RM&R referrals Collaborate with unit clerk to resolve short notice staffing issues Just-in-time training and education for PCU and PACSI teams Patient intake and general support for PACSI clinic patients Collaborate with clinical manager to monitor and manage resource and medication utilization Collaborate with manager for patient safety incident reviews Co-lead and/or champion quality improvement projects Other duties as assigned

Qualifications

Current registration with The College of Nurses of Ontario Completion of the Pallium Canada LEAP Foundations Certification A minimum of two (2) years recent RN experience with Palliative patients in inpatient and outpatient settings required. Recent experience in medicine, surgery, ICU, ER and/or oncology required Proficiency managing port-a-catheters, PICCs, IVs, and blood draws Completion of or willingness to complete the Hospice Palliative Nursing Certification (CNA) required Strong organizational skills to manage competing priorities Ability to prioritize and activate relevant support structures for resolution of day-to-day operational issues. Experience in bed management, resource utilization, and referral triage. Role model with demonstrated leadership and team building skills. Demonstrated ability to work cooperatively in an interdisciplinary team environment. Demonstrated critical thinking and problem-solving abilities. Experience developing and implementing standard operating procedures (SOPs). Demonstrated skills in complaint management and conflict resolution. Demonstrated commitment to professional development and learning. Recent leadership experience required. Excellent written and verbal communication. Computer skills required (Word, PowerPoint, EPIC, Excel and Email) BLS certification required. Excellent Attendance Record.
