The Opportunity



We are looking for a

Team Lead, Compass Housing

to join our

Housing Team

at CUPS on a one year fixed term contract.

Compass is a multidisciplinary mobile housing team that provides coordinated in-home intensive case management services, and prioritizes clients with complex issues who are living with interrelated mental health and addictions concerns.

The Team Lead, Compass Housing will be responsible for providing daily mentorship and guidance to the Compass team under the leadership of the Housing Manager. You will ensure that the daily operations of the programs are delivered to clients per CUPS service delivery models of Integrated and Trauma-Informed Care. You will be integral in the success of the CUPS service program through communicating and aligning team action with organization goals, supporting the effective performance of the team and maintaining a practice of direct client service.

Key Responsibilities



Maintain oversight of all Compass service delivery to ensure the provision of service per CUPS policies, procedures, service model frameworks and program goals. Cultivate a culture of integrated care, collaboration and commitment to organizational values. Provide daily direction and coaching to the Compass teams members, including regular check-ins, team meetings workload planning, regular coaching, and performance management. Deliver integrated client service through cross-departmental communications, multi-disciplinary care planning and information sharing across departments. Supporting workers in identifying opportunities to engage additional CUPS services to provide integrated services to clients. Provide leadership and coaching to assist staff with the achievement of an integrated and team-based model of care for CUPS' clients, identifying process and skills gaps to develop improvement plans in conjunction with the Housing Manager. Provide care planning, client engagement coaching and direction to team members as needed for worker development and optimal client outcomes based on Brain Science, Trauma-Informed Care and Integrated Service delivery. Act as warden for client dignity by ensuring confidential client information is handled with the utmost care and respect in alignment with privacy protocols & CUPS values. Conduct file and database audits of Compass as part of CUPS continuous improvement strategy to ensure holistic and accurate documentation based on standard procedures of CUPS integrated care tools. Provide direct client service on a modified schedule within Team Leader duties, both alone and with colleagues in a variety of settings including in private homes, shelters, on the street, in camps, in office and while transporting clients in personal vehicle. Understand program evaluation & reporting requirements, ensuring data integrity & availability for evaluation teams, producing program reports as needed and requested.

Employment Requirements



Five or more years of experience working in housing, addictions or similar field. Demonstrated experience operating in a leadership capacity and established skill in coaching and supervision. Current CPR & First Aid certification. Current and valid Drivers' License, with access to own personal vehicle.

What You Bring



Previous experience working in multidisciplinary and integrated care models and asset. Knowledge and integration of trauma-informed practice in both client and supervisory approach. Alignment with Housing First philosophy. Ability to be self-directed and exercise sound judgment. Brain Story Certification an asset.

Work Conditions



Exposure to trauma and emotionally complex cases. Technology use, including CHR Electronic Records System, MS Office 365 Suite. Periods of stress due to work volume and work pace. Manual dexterity required for computer use. Individual and collaborative team-based work. Intermittent periods of sitting, standing, walking, driving.
