Carefirst Seniors & Community Services Association
is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.

Team Assistant
to join our team.

Essential Duties and Responsibilities:
Working closely with an interprofessional team, the Team Assistant will provide: reception, clerical/coordination, and administrative support to the mobile care team
Engage with community members from diverse backgrounds to support integrated primary health and social care and health promotion functions
Collect and track and help analyze statistics for trending and reporting and program evaluations
Actively participate in the development, implementation, and evaluation for this innovative model of health promotion and wellness clinics in the community
Other duties as assigned in support of operations
Qualifications & Job Requirement:
Post-secondary education in a relevant field (e.g. office management, health care) an asset
Experience with community-based outreach in an employment or volunteer capacity
Valid Ontario Driver's license and access to a vehicle
Demonstrated current expertise/experience in EDI and anti-racism practices, integrated primary care and management, chronic disease prevention and management, health promotion and wellness.
Fluency in a second language that reflects the communities that we serve
Preferred skills: communication, computer skills, customer services, organization, time management, problem-solving, working independently and as part of a team
Proficient in various office applications, such as Microsoft. Experience in EMR a strong asset.
A valid vulnerable sector police record check completed within one year
