Permanent, Full-time (35 hours per week)



$64,727.50-$73,294.38 + Health Benefits + HOOPP

BACKGROUND



The Vaughan Community Health Centre (VCHC) is a not-for-profit, community-governed organization providing primary health care, social services, and illness prevention programs primarily to the residents of the Western and Northern York Region. We serve people experiencing barriers to accessing health care services due to culture, language, age, chronic illness, or the like. We are committed to providing services with and for local Indigenous, Black, racialized and other disadvantaged communities.

VCHC has implemented a direct referral system to improve access to inter-professional care by allowing non-team-based family physicians to connect vulnerable (medically and socially complex) clients to services available at the VCHC. As part of the health promotion framework of our services, we work in partnership with other community-based health and social service organizations to address individual, family and community needs.

POSITION SUMMARY



As a member of the interdisciplinary team, the System Navigator (SN) supports clients in meeting their health care and social needs by connecting them to health care and social systems.

The SN delivers culturally and linguistically responsive services that meets the needs of clients living in Western and Northern York Region, including local underserved communities (e.g. Indigenous, Black, 2SLGBTQIA+, etc.). The position has a strong emphasis on the social determinants of health framework.

KEY ACCOUNTABILITIES AND RESPONSIBILITIES



Collaborate with internal and external health care providers (i.e. Physicians, Nurse Practitioners, Allied Health Professionals, Community Health Workers, etc.) to support mutual clients in meeting their health care and social needs. Assess the social determinants of health of individuals/families and co-create personalized plans that reflect the client's needs and social circumstances, empowering clients to play an active role in their health and wellbeing. Support clients in meeting the needs of their personalized plan via referrals and warm transfers to service providers. Offer hands-on navigation support for clients with more complex care and access needs. Conduct check-ins to support clients in their journey of accessing services and meeting their needs. Identify arising barriers and provide support in removing those barriers. Advocate on behalf of clients with external parties for access, resources and support. Establish a rapport with clients and create an atmosphere in which clients can open about their needs. Establish relationships and connections with key stakeholders (i.e. internal/external health care providers, community organizations, caregivers) to create a cohesive support network for clients, facilitate resource sharing, and foster mutual trust and understanding to facilitate warm transfers for clients. Enter client encounter data on the VCHC Electronic Medical Record. Maintain and safeguard case notes.

QUALIFICATION REQUIREMENTS



Bachelor of Social Work or related social services field. Minimum of two years related work experience in a social services or community-based health care setting delivering culturally and linguistically responsive and safe care that meets the needs of local population, including underserved communities (e.g. Indigenous, Black, 2SLGBTQIA+, etc.). Proven experience in Person-Centred care planning that tailor supports to individual's needs. Application of anti-racism and anti-oppression principles in client service delivery. Strong understanding of key social determinants of health affecting individual and community wellbeing. Excellent communication and interpersonal skills that build and maintain strong relationships with stakeholders. Proven experience with strategies such as coaching and motivational interviewing to understand clients' readiness for change and encourage behaviour change. Demonstrated ability to build and maintain relationships with external service providers for the purposes of client referral. Deep understanding of health and social services available in York Region and how to access them, including proficiency in navigating government benefits, healthcare systems, and other resources to connect clients to appropriate supports. A track record in building trusting relationships with clients to meet their needs. Proficiency in MS Office, electronic medical records (EMR) systems and other healthcare technology commonly used in Ontario's primary care setting. Demonstrated ability to work effectively in a multi-disciplinary team environment. Proficient in a second language that reflects the diversity of the communities served by VCHC is an asset. Valid driver's license, regular access to a vehicle and appropriate insurance for personal automobile.
