Job Description

OVERVIEW




Michael Garron Hospital operates several services that operate offsite in East Toronto:

Women's Withdrawal Management Services (989 Danforth Ave.) Men's Withdrawal Management Services (985 Danforth Ave. Community Outreach Services (671 Danforth) Mobile Crisis Intervention Team
These facilities provide supports individuals that are impacted by serious mental health concerns, many with concurrent disorders, and their families. WMS provides compassionate and culturally competent spaces forhealing and respite for individuals aged 16 and older who are in an acute state of intoxication or withdrawal from alcohol or other substances. The team at COS is comprised of the Reach team, the Community, School & Family Outreach Program and COMPASS, the Assertive Community Treatment Team (ACTT). COS is a community-based mental health program providing interdisciplinary care using a flexible, responsive approach to ensure individual's needs are met in a timely, client-centred manner and with minimal barriers to access.



Both our Women's Withdrawal Management Services and Men's Withdrawal Management Services are open 24/7. They are open to individuals who self-refer or who are referred by a clinician or member of the community.



The ideal candidate is committed to providing high quality, evidence-based care in a fast paced and developing clinical environment where roles and responsibilities are continually evolving to better meet the needs of our clients. Reporting to the Manager the Supervisor functions as an active member of the inter-professional leadership team and work closely with the Management and Administrative teams to provide supervisory leadership to ensure care is delivered using a health equity lens and according to best practice, relevant College requirements as well as applicable program standards. Monitoring and supporting optimal care through regular supervision, the Supervisor offers leadership, mentorship, training and education opportunities for all members of the team.



This position will be required to work across multiple sites and may include some evening/weekend and on-call work.





PRIMARY RESPONSIBILITIES



Oversee and support daily operations of the respective program sites, including scheduling of staff and management of onsite resources/ supplies. Ensure adequate staffing coverage at all program sites, including scheduling of staff and responding to emergent needs. Lead, support and facilitate team huddles/discussions and care planning to ensure optimal team functioning and the provision of high quality, interdisciplinary care that aligns with current best practices in substance use and mental health care. Collaborate with team members to develop, monitor, review and evaluate comprehensive,client centered, individualized care plans. In collaboration with the program Manager, develop and implement strategies to support optimal team functioning to ensure excellence in inter-professional care and a positive work environment. Direct andcoordinate the seamless flow of clients within and across programs including intake, admission, assessment, referrals and discharge. Collaborate with the programs and services of the East Toronto Health Partners to facilitate timely, relevant and person-centered services for individuals impacted by addictions/substance use and mental health. Ensure program sites operate in accordance with all health and safety requirements and offer a safe, inclusive, respectful and welcoming space for clients. In collaboration with the respective Manager, participate in all aspects of program staffing, including recruitment, supervision, orientation, evaluation and performance management. Monitor the reporting of program incidents using a quality improvement lens to minimize risks and to identify opportunities for improved service delivery and care. Respond to community concerns and complaints in collaboration/consultation with respective Manager. Participate in the collective bargaining process, grievance procedures, and other committees as a member of management team. Conduct regular meetings with individual staff to review their work with clients, assess clinical performance, address challenges and provide support/feedback. Oversee completion of all required documentation in a timely manner and ensure it meets all organizational, legislative, and professional requirements where appropriate. Work with Manager(s) to develop and implement a quality improvement workplan to ensure program adherence to relevant standards. Develop, maintain, review and revise program policies as necessary. Foster positive relationships with relevant community partners and organizations to respond to emerging community needs and to support an integrated, comprehensive approach to client care. Ensure programs meet targets and benchmarks in accordance with funder and organization requirements. Create a climate of continuous learning and professional development, including the identification and coordination of team-based training opportunities. Share On-Call responsibilities.



SECONDARY RESPONSIBILITIES



Represents the department on hospital committees and represents the hospital on community-based committees. Knowledgeable aboutthe Standards of Practice of many disciplines. Create and maintain a safe and healthy work environment. Support a culture of innovation by embracing diversity, creativity, change and dialogue. Model a willingness to be open and receptive to newideas and ways of doing things.


EDUCATION




University degree in a health care related field required, Bachelor of Science in Nursing strongly preferred. Current registration with respective College, i.e. College of Nurses of Ontario. Demonstrated knowledge of all relevant legislation, including the Mental Health Act and PHIPA. Demonstrated knowledge/training in program management, team leadership and clinical supervision. Advanced training/diploma in Addictions and Mental Health from an accredited College an asset. Advanced training/knowledge in arrange of therapeutic modalities and approaches to substance use and mental health, crisis prevention/intervention, harm reduction principles, strength-based, trauma-informed and anti-oppressive practice an asset.




QUALIFICATIONS




Minimum five (5) years experience working with adults living with mental health and substances use challenges in a community or hospital based setting. Minimum three (3) years experience in a clinical leadership/management role. Experience working in Withdrawal Management Services is a strong asset. Demonstrated experience in previous program development, administrative and supervisory experience. Experience working with adults living with substance use/addictions and concurrent disorders. Proven expertise in evidence-based therapeutic approaches to substance use that are client-centered and strength-based, from harm reduction to abstinence-based models. Experience preventing and responding to crises. Demonstrated competency andexperience working with diverse communities using a health equity lens. Demonstrated ability to liaise effectively and operate collaboratively with community and hospital-based stakeholders. Sound knowledge and experience working with adults with histories of trauma, abuse, self-harm and/or suicidal behaviour. Required Knowledge of the Mental Health Act and other relevant legislation related to substance use/addictions. Strong ability to work/lead in an interprofessional team environment. Willing and capable of making time-sensitive decisions to ensure the safe operations of all program sites. Strong oral and written communication skills. Excellent interpersonal skills including conflict management and de-escalation techniques. Proficiency in the use of computer technology and various software applications. Good work and attendance record required.
