The opportunity

We are currently looking to fill

one new opportunity for a Full-Time Stroke Community Navigator

position in our Post Stroke Transitional Care Program in Timmins, ON

Starting wage:

$31.12 per hour

Hours:

35 hours per week

Responsibilities

Responsible for enhancing the community reengagement of stroke patients returning home following discharge from hospital. Coordinate the access, service provision and community referrals as part of March of Dimes' Post-Stroke Transitional Care Program. Be part of a multidisciplinary team, provide outpatient assessment and community re-engagement services

Qualifications and experience

A member in good standing with a professional college and/or member of the RHPA Related experience working with stroke survivors in the acute, rehabilitation and/or community care aspect of the healthcare continuum Extensive knowledge of Sault Ste. Marie and area based community services Knowledge of Stroke and the Ontario Stroke System Knowledge of March of Dimes Canada Case management experience Program evaluation and outcome measurement experience Exemplary interpersonal and communication skills, both written and oral in English Must have a valid driver's license and personal vehicle French-English bilingualism an asset
