Horizon House is a Community Residential Facility (CRF) contracted by Correctional Services of Canada (CSC). We operate as a federally funded "halfway house", providing 24-hour supervision, structure, and reintegration support to individuals transitioning from federal institutions back into the community.
We are currently seeking a Specialized Support Worker to join our Case Management Team. This role is well suited to a highly organized and adaptable professional with experience supporting individuals with complex needs in justice-involved or high-risk environments.
The successful candidate will be self-motivated and driven, with a strong interest in program development and a demonstrated ability to apply client-centered, strengths-based approaches in their work.
About the Role
The Specialized Support Worker plays a key role in ensuring continuity of care, program safety, and successful reintegration outcomes for residents. While maintaining core Support Worker duties, this position also provides enhanced coordination, intake oversight, and specialized support in collaboration with case management staff. This position reports to the Program Manager and works closely with the Case Management Team.
Key Responsibilities
Provide direct support to residents with complex needs, including referrals, coordination of services, and continuity of care
Complete daily facility operations such as hourly rounds, resident sign-ins/sign-outs, collateral checks, and detailed documentation
Manage client intake referrals and oversee the In-House Identification Program, supporting access to ID and community resources
Participate in pre-release coordination, including responding to inmate correspondence and participating in in-reach calls
Collaborate with the Program Manager, Program Assistant, and Case Workers to address complex or unique intake and transition situations
Support facility safety and operations by participating in Occupational Health & Safety activities, maintaining emergency documentation, and supporting fire drills and safety checks
Complete monthly program and financial reporting, including ID program updates and expense tracking
Qualifications
Post-secondary education in Social Work, Criminal Justice, Psychology, Human Services, or a related field
Experience working with justice-involved or high-risk individuals in a residential or community-based setting
Strong documentation, communication, and crisis-management skills
Ability to maintain professional boundaries and follow structured policies and safety protocols
Proven ability to work effectively within a multidisciplinary team
Knowledge of federal corrections and community reintegration processes is an asset
Hours & Compensation
0.8 FTE (32 hours per week)
Shifts may include evenings, weekends, and holidays
Start date: January 15, 2026
Wage: $24-$27/hour, based on experience
